2 Licensing Before you even look for applications, contact Christy Gass or Gabby Lane and confirm the licensing/appointment is in order.

3 Forms All carrier forms are on the FHC site.
Log into the FHC website to obtain –Insurance (applications paper) – complete the IPIPELINE questions and Search.

4 Forms Always send all pages of all forms.
If there are 3 pages to a replacement form and you only completed information on 2 pages…all 3 pages must be returned.

5 Replacement Forms Every application will ask about replacements.
If replacement is involved you HAVE to complete the replacement form. The replacement form HAS to be dated on or before the application date. Without this, you risk the application being rejected and all new forms resubmitted. If you are in an NAIC Replacement state and you have coverage inforce and you are not replacing, you still HAVE to complete the replacement form and you HAVE to have it dated on or before the application date.

6 Pictures/JPEG’s Do not send pictures of forms.
These do not image well and are often not accepted by the carrier. Don’t tell me that it is clear and you can read it – they don’t care.

7 Illustrations Always submit an illustration with every application. This will help avoid delays. New business always requires an illustration to be completed for our system. Permanent coverage requires a signed illustration prior to issue. ALL PAGES MUST BE RETURNED

8 Submit to Treeno Upload your applications, illustrations, forms and various requirements to Treeno. ALWAYS send an to Julie letting her know after you have uploaded so she can make sure the image is received.

9 InStat We enter all new business in InStat.
Agent letters generate from this system. Our Status Reports come from this system. Daily pending

10 APS orders APS – Attention Physican Statement
FHC orders using a vendor – EIS. Orders placed on-line and status’ done by the vendor Doctors may use a vendor, may restrict status updates and may copy records themselves. Proposed Insureds are welcome to contact the doctors to push this along.

11 XRAE Link on the FHC website
Great for build and questionable tobacco users.

12 INFORMALS FHC PRE Must have to shop any information
Agent can provide summary w/o any identifying info w/o a PRE Auth

14 Expect Problems… The e-application process – short form, is done to allow the carrier to do most, if not all the work. The carrier will order the phone interview. The carrier will order the medicals. The carrier will order the APS’. If you are shopping for the best offer, or to get 2 term policies, do the full applications/long forms…avoid multiple exams, phone calls and APS orders.

15 Principal Accelerated Underwriting
Complete all normal submission forms with the exception of the Part 2. Call and schedule your clients interview by calling 888-teleapp ( ). Answer Agent Page with YES Accelerated Underwriting and list the confirmation code for the interview you just ordered.

16 Principal Accelerated Underwriting
Products: Term, UL, IUL, SUL Ages: 18-60 Face – up to $1 million Risk Classification – Super Preferred and Preferred. Standard for ages 18-19 Limitations – No major medical conditions, no MVR issues, no criminal history, no foreign nationals or travel to hazardous countries, over age 50 – have to have a PCP w/ evidence of routine care/physicals, no prior informal with Principal w/in the last 24 months, no labs completed for insruance purposes within the last 12 months, can’t have a prior approval other than preferred or super preferred. If denied Accelerated Underwriting – can still get best class. Random checks – random cases pulled as a way to audit the AU process.

17 Prudential EXPRESS Long form – you ask the medicals questions
Express – a phone interview is done to complete the Part 2 *typically carriers that conduct a phone interview will do an Abbreviated Paramed with the labs, verses a full Paramed. This makes a difference when you order the medicals for Prudential.

18 Pacific Life Term Must complete the term worksheet
Pacific will conduct a phone interview once the file goes to underwriting. *do not call prior to this order Pacific will order the medicals Pacific will order the APS’ If submit w/ a UL…all hell breaks loose

19 Cover Letter Nothing fancy Purpose of coverage
Any medical history to include doctor names, address, phone for contact, date last visit and why helpful. Include any specialist the client may see. Note if the case is part of a group or family already pending in underwriting. Note if one of the case managers is already working on this one.

20 Helpful Hints If you send the delivery requirements direct to the carrier, let us know and send copies to treeno. If you order the medicals for your cases, tell the examiner to fax copies to FHC at Coverage inforce section of the application…all policies inforce must be disclosed, even if they are not being replaced.
