On-Going Service Provider Contract
Initial One-Year with Five (5) Option Renewals Review Contract which is included in RFQ – no revision of terms permitted Successful respondents of this round need not submit for future rounds scheduled later in 2017

HUB Documentation Section 5, which includes the HUB documents
HUB documents for submission (required for RFQ) No sub-consultants considered for RFQ submission Failure to submit required HUB documents will result in rejection of the Request for Qualifications HUB Plan on per project basis for contracts $100K and over (if selected) Exhibit H included in RFQ for future use on per project needs

Submission Main Points
Requirement Section in RFQ Submit two (2) separate .pdf document submissions each for Section 4 and Section 5 Section 3.2.1 Size 15MB limit for each .pdf Use Firm Naming Standard Section 3.2.5 Section 4 and Section 5 responses shall include required Adobe bookmarks Section & Section 3.2.4 submissions must be from the same address within the firm Section 3.3.1 Submit both sections separately to Section 3.3.3 PMCS acknowledgement of submission receipts Section 3.3.4 Failure to submit per RFQ requirements shall disqualify for consideration Section 3.1.7 Highlight in RFQ
