Approaches used: Informal Transfer of older persons from the LTC to the Geriatric centre and vice versa. Co-ordination: both parties. Geriatric centre through the MSW unit LTC: through the administrator.

REFLECTIONS Positives: Challenges:
Geriatric centre was involved during the construction of the care home. Available for all clients irrespective of the religion, creed & diversities The founder has experience with care homes. The care plan, fees etc well spelt-out and available online [ Challenges: Formal agreement between the LTC & Geriatric centre is on-going Logistic problems
14
IMPACTS ON OLDER CLIENTS
Positives: Good care plan (well spelt-out) Integration of cultural values in care management. Good healthcare integration with Geriatric centre. Clients are involved in decisions on their care. Not so positives: Logistics
15
THE FUTURE Completion of the formal agreement
More training collaborations. Key lessons: LTC is an integral part of the care of older persons Both parties have some form of training in the care of older persons The LTC was registered with Government.
16
ROLES OF STAKEHOLDERS Government:
All LTCs should be registered & monitored by the Government Carers of older persons should receive formal training (Certificated courses) from accredited institutions. Establishment of the care commission for the older persons. Older persons’ bill should be passed & signed into law
