Image Challenge Q: An 86-year-old woman with hypertension presents with epigastric pain and does not report any respiratory symptoms. Her chest radiograph demonstrates these findings. What is the diagnosis? Answer: Oleothorax The correct answer is oleothorax. This patient had a history of pulmonary tuberculosis in the 1950s, and treatment at this time for this condition involved instillation of oil into the pleural space to collapse the involved lung. The oil was usually aspirated after the treatment was complete, which was usually a period of two years. In this case, the patient was lost to follow up after completing the treatment and the oil was left in place.
