During Fall 2016, the Division hosted a series of Family Forums and more focused stakeholder meetings to gather feedback on ways to improve the PCP/ISP approach The Division “set the stage” by sharing information on the LifeCourse Framework and by using the trajectory tool to facilitate the discussions. Held over 50 presentations that reached well-over 1,000 individuals

Alignment with LifeCourse Principles “help[ing] individuals and families of all abilities and all ages develop a vision for a good life, think about what they need to know and do, identify how to find or develop supports, and discover what it takes to live the lives they want to live.” Reset Case Management Vision and Expectations person-centered focused on identifying needs and preferences, navigating resources and connecting to supports aligned with their needs. Defined Case Managers Role as: An expert navigator; An advocate; and A partner in the process
