KNEE OVERUSE INJURIES Tal S. David, MD SportsMed Surgery
2
What Is An Overuse Injury?
Overuse injuries = Repetitive strain to tissues “Too Much, Too Fast” In children, this rarely occurs in free-play activity, common in organized sports
3
“Kids are not little adults”
4
Pediatric Athletes Are Unique!
Skeleton is immature Growth plates are open “weak link” Year-round play at young ages Skill levels at greatest discrepancy
5
Overuse Injuries The Pediatric athlete is UNIQUE:
Growth spurts play a role BOYS GIRLS Reach Puberty Later Earlier (1-2 years sooner) Grow taller and heavier (12-14 yo) Reach Adult Height 18yo 16yo
6
KNEE No joint in the body is more affected by overuse injuries that the knee
7
Osgood-Schlatter Disease
8
Osgood-Schlatter Overuse injury to the attachment of the patellar tendon The most common overuse injury of the knee
9
“Bump on the Knee”
10
Osgood-Schlatter Tender to the touch / limited flexion
11
X-Ray of Osgood-Schlatter
12
Patellar Tendinitis “Jumper’s Knee”
13
Patellar Tendinitis Associated with: Occurs during a period of
Repetitive jumping An increase in activity Occurs during a period of growth acceleration
14
Patellar Tendinitis Inflammation of the patellar tendon
Tender to the touch Stiff and limited flexion due to pain Generally no swelling Slightly older kids – Growth plates closed
16
Treatment: Overuse injuries respond well to REST Activity modification
Short term anti-inflammatory agents Ice Stretching
17
Treatment: OK to resume normal activities once the symptoms resolve
Return to play varies (chronicity)
18
SUMMARY: Overuse injuries occur because of repetitive stress
Kids are uniquely predisposed to these problems Knee is most commonly affected joint These injuries respond well to rest Early treatment can shorten recovery time
