3. C3RS Implementation : M of E signed the IMOU on January 19, 2016
Management and union reps met with the FRA on March 1st, 2nd, and 3rd for PRT training
The week of April 11th 2016, C3RS was rolled out to participating members
The “Go Live” date was April 18th, 2016

4. Benefits of C3RS Complements MNR existing programs – IR1/IR2 process
Builds a positive safety culture – Operation Deep Dive identified a poor safety culture at MNR
Proactive (early warning system)
Focuses on problems, not people
Provides incentive for learning from error
Targets the root cause, not the symptom(why are things the way they are)

There have been some near misses reported
The Majority of the cases that have been submitted
Included the employee identifying a cause or causes of the cases when filed with NASA.
Employee failed to follow operating procedures.

Real-time observations
Situation Awareness reminders discussed during JSBs and Safety Focus days
We have had some safety concerns on general work practices.
Responding to those conditions we enlisted the safety department to conduct a JOB SAFETY ANALYSIS on the task.

7. There has been collaboration with our Transportation counterparts, which has been very helpful
We plan on continuing meeting with our counterparts for any shared cases
The M of E team is enthusiastic and meets monthly based on our current caseloads

8. M of E Corrective Actions Implemented:
Placement of mirrors in yards with blind spots making train traffic difficult to see at road crossing
This structure creates a blind spot when attempting to cross

9. M of E Corrective Actions Implemented:
Addition of the mirror allows fork truck operators and other employees to see oncoming train traffic heading towards the roadway
New Mirror installed

The PRT Group is watching this area since it is under construction to make any correction if needed.
