Service Provider Note Purchase Agreement
This agreement defines the “terms” established for patient notes.

Once MyLoans agreement has been finalized, your facility will be setup with user access to the website.

Patient Qualifications
No credit check or approval process Required information to start process: Social Security Number Authority to sign patient note Co-Signer or Co-Borrower, if necessary

Reassignment Once 60 days past-due the patient note is reassigned
