Summary of changes taking effect January 1, 2018
This will be Union’s first Open Enrollment in the fall. Open Enrollment will occur each fall for changes to take effect January 1st. The changes you make now will be in effect January 1, 2018 through December 31, Deductibles for the medical and dental plans will continue to be on the calendar year and the vision plan will change to a calendar year benefit beginning January 1, 2018. Calendar Plan Year Effective January 1, 2018 the TPA will be UMR instead of HealthSmart. New medical ID cards will be issued prior to January 1st. All standard copays, deductibles and co-insurance will remain the same. PPO Plan - Changing Third Party Administrator Along with a new TPA there will also be a new provider network. The new network will be UnitedHealthcare Choice Plus. Moving to this network should cause minimal disruption. The new provider directory is available at PPO Plan – Changing Network Continue

The LabCard program will be ending December 31, Quest Diagnostics and DLO will no longer be in-network. Beginning January 1, 2018 all in-network covered outpatient lab work will be paid at 100%. In-network providers include Regional Medical Lab (RML) and LabCorp. LabCard Program The MyDecision Program currently administered by HealthSmart will change names to the MyChoice Program. Oklahoma Surgical Hospital will administer the program and will also be in-network. All services and treatment at Oklahoma Cancer Specialists and Research Institute will continue to be paid at 100% through the program. MyDecision Program Effective October 1, 2017 – 100% plan paid oral appliances for mild to moderate sleep apnea. (918) Riverwalk Dental Spa – Oral Sleep Apnea Devices Continue

NO RATE CHANGE In Network Individual Deductible $250 90% In Network Coverage $2,000 In Network – Out of Pocket Max $25 copay -Primary Care/Specialist Office Visit $50 copay - Urgent Care 100% paid outpatient lab work for covered labs. Envision Imaging provides MRI’s, CT scans and x-rays at a reduced cost. The MyChoice program is a voluntary benefit that members can utilize to receive covered surgery, medical services and cancer treatment with no co-pays, co-insurance, or deductible costs. This program is available through Oklahoma Surgical Hospital and Oklahoma Cancer Specialists and Research Institute. $5 copay for generic prescriptions $35 copay for brand name prescription 20% copay for prescriptions costing $1,000 or more (30 day supply) Coverage Level Monthly Premium Single $0 Employee + Spouse $555 Employee + Children $300 Employee + Family $700 Continue

Vision Plan – VSP Dental Plan – Delta Dental of OK Exam & Materials Plan – Includes an eye exam for a $10 copayment plus a materials allowance for glasses ($25 copayment) or contacts. Qualifying Events Open Enrollment is your opportunity to makes changes to your current elections. These changes go into effect January 1, 2018 and stay in effect until December 31, 2018. The only way to make changes mid-year is with an IRS defined qualifying event, including, but not limited to: marriage, divorce, birth/adoption of a child, death of a spouse, losing or gaining other medical coverage. Should you experience a qualifying event, please notify the Benefits Office within 31 days to make any appropriate changes. If the Benefits Office is not notified within 31 days the law prohibits you from making any changes until the next available Open Enrollment or upon experiencing another qualifying event. Continue
