Background COPD is a leading cause of mortality and morbidity in the UK In 2012 COPD admissions accounted for over 1 million bed days in the UK (BLF 2016) People with COPD are limited by breathlessness but also experience lower levels of mental well-being when compared to age-matched populations. Pulmonary rehabilitation is proven to increase exercise capacity and health status but uptake is limited (BTS 2013)

The first RIPPLE: Partnership from the outset
A prototype based on a co-created theory of change An asset-based community development approach to improve wellbeing and reduce the high levels of anxiety and social isolation observed in individuals with severe Chronic Obstructive Pulmonary Disease (COPD) in Coventry People with COPD, their carers, respiratory nurses, consultant respiratory physician, public health, voluntary agencies, the University

Once weekly community clinic Run by Chaplaincy from UHCW, Grapevine and People Point Input from clinical team, Age UK and British Lung Foundation Activities include self management advice from respiratory team, seated yoga, mindfulness, bingo, quizzes

Making Waves whilst creating partnership
Health Foundation Funding ~£500K : spreading improvement programme Externally - Working with partners in Coventry, Dudley, Northamptonshire, Nottingham North East, South Birmingham, South Lincolnshire and Wolverhampton. Internally - Working inter-faculty (School of Health, School of Life Sciences, School of Psychological, Social & Behavioural Sciences, Centre for Technology Enabled Health Research and Professional Services) and cross-faculty with the Faculty of Arts and Humanities.

Complex Evaluation Evaluating how the project improves the health and wellbeing of patients with COPD, their self-efficacy and ability to self- manage and the impact of integrated support on individuals with long-term conditions, communities and the healthcare system Exploring the barriers to and enablers of innovation in complex healthcare systems, in interagency working and in the NHS Branding, communications and impact plan to enable the review of the effectiveness of communication of innovation and improvement within the NHS
