1 School Notebook This power point will take you step by step through assembling your notebook. Inside will be all your Documentation including: Fee Form; check; Inventory Form, Art teacher/school rep. contact info, and Image Information/Model Release Forms

2 Schools will assemble a notebook with the following included in it:
Signed and completed Artwork Information Forms for each work entered. Signed and completed Model Release Forms as needed. Signatures of both Student and Techer. Teacher or Representative contact information for the duration of the Competition Inventory Forms f. Fee Form g. Fee Check Schools will be provided with all labels needed in the form of a barcode label at registration. Schools will be able to display their artwork for the competition once they complete registration with all proper documentation. j. This notebook will be turned into TAPPS for the duration of the competition.

Let’s start with a plain 3 ring notebook. At Walmart for $ Make sure the notebook has a plastic sleeve in the front and on the spine. Next hand write or type out on a piece of 8 ½ x 11 white printer paper the following centered for the front of the notebook: Name of School –( big font – 60 should do it). City Located In Division (1A,2A,3A,4A,5A) Teacher/School Representative contact information The same information needs to be hand written or typed onto another piece of 8 ½ x 11 white printer paper that goes into the spine, If you decide to type it, opening a new document in PowerPoint and creating a slide will make it easier to get the right size for the spine.

4 Next purchase a 5 set of dividers – again Walmart has some for $1.79.

5 1ST DIVIDER Write on the divider tab: Inventory Form Then log into TAPPSter and print out the Inventory Form and three hole punch. You will be able to access that once the state meet closes. (approximately 2 weeks prior to competition)

6 Write on the divider tab: Fee Form with Check
2nd DIVIDER Write on the divider tab: Fee Form with Check Then log into tappster and print out the Fee Form and three hole punch. You will be able to access that once the state meet closes. (approximately 2 weeks prior to competition) This is where you would put your school check - paper clipped to the back of the Fee Form. Remember to include the costs of Senior Portfolio, Art History, and Short Film. And be aware we do not give receipts out. If you need verification that you turned in your money please print another Fee Form and the Registration volunteer will sign it as proof.

7 3rd DIVIDER Write on the divider tab: Contact Information Provide the teacher/school representatives contact information for the duration of the State Meet. This can be typed or hand written on a 8 ½ x 11 piece of white printer paper. Three hole punch.

4th DIVIDER Write on the divider tab: Image Information/Model Release Forms On the tapps.net webpage under forms you will need to print out enough Image Information/Model Release forms so that every work of art you are bringing with you has one. Make sure you have also completed the Model Release part if necessary. Student and Teacher must sign. Three hole punch. If your students work is not from imagination you will need to include public domain paperwork (the print out from the internet must say public domain). And/or reference photographs. Additionally if you have any other documentation you need to include please paper clip all of it to the back of the Image Information Form. NOTE: At the top right of all your Image Information Forms please put the title of the work and the student’s name if it is not on the form somewhere else.

9 NOTE: If your students work is not from imagination or from a student taken/reference photo then you will need to include public domain paperwork (the print out from the internet must say public domain). See below for suggested Public Domain Sites: Publicphoto.org

10 5th DIVIDER Write on the divider tab: Miscellaneous This tab could be used for notes that the teacher wrote, map to location, paperwork for workshops, etc…

11 At Registration Schools will be provided with all labels. Schools will be able to display their artwork for the competition once they complete registration with all proper documentation. This notebook will be turned into TAPPS for the duration of the competition and returned at the end of the competition with awards.
