2 Logging onto the 7 Day Self Assessment Tool

3 Trust from the drop down list
Select your Trust from the drop down list Enter your password and Click ‘Log In’ If you are having trouble logging in or registering, please get in touch using this button, or contact NATCANSAT on or us: User Guides & Supporting Resources to help you through the process will via these two buttons.

4 Entering trust data into the tool

5 Once you have logged in you should see this Home Screen
Click the ‘Enter Data’ button to access the data entry screen to then start entering your survey data

6 There are 5 tabs across the top of the page that need data entering
Enter the number of emergency admissions, and exclusions then click this button to calculate your case note review sample size Hover your mouse over here for more explanations (and see Supporting Information in resources section) Click “Save” or “Next” to save your record and move to the next tab. All questions on this page are mandatory and will need completing before your data can be approved.

7 Click the tick box if your Trust has no Inpatient beds for that specialty.
If your Trust has inpatient beds for this specialty please select “Formal Arrangement” Click the drop down box to select the level of Provision your Trust has for each specialty All drop down boxes for “Level of Provision” on Question 1 and Question 2 on this page are mandatory and will need completing before your data can be approved.

8 If your Trust has not participated in the local survey click here
This section gives details of your local survey that your Trust will need to carry out to enable you to complete this tab. If your Trust has not participated in the local survey click here Enter the total number of Consultants If your Trust has completed the local survey, these two items will be completed for you (by Friday 30th September)

9 Consultant Directed Diagnostics
If your Trust has completed the local survey, part 2) and 3) will be completed for you (by Friday 30th September) To help you answer Q4, you will be able to download your Trusts responses to your local survey (by Friday 30th September) 4

10 This is the same link as on the Diagnostics tab
This is the same information as on the Consultant Directed Diagnostics tab, and if this is ticked on one tab, it will also appear on the other You only need to enter this once either on the Diagnostics or Interventions tab.

11 Consultant Directed Interventions
If your Trust has completed the local survey Q2 will be completed for you (by Friday 30th September) To help you answer Q3, you will be able to download your Trusts responses to your local survey (by Friday 30th September) Click the drop down box to select the type of arrangement for each intervention. All responses are mandatory for Q4 and will need completing before your data can be approved.

12 Before you can save a record you need to select the admitting Hospital Site. If the required site isn't in this list, please use the “Contact Us” button to let us know the Site Code and Name that needs adding This will then generate a patient ID. You will need to keep a local reference as to which patient this refers to. Your list of patients will appear here when saved. The list will be in Hospital Site then numerical order

13 You can search for previously entered patients here
You can save, delete or add patients here

14 When you click a date field, a calendar will appear for you to select the date.
If the time is not documented, please tick this box (this will mean you will fail this standard for this patient) Once you have entered the arrival date and discharge date, this will open a series of subsequent days for you to enter data for. These dates are auto populated depending upon what you select for the arrival and discharge date

15 If you enter an invalid date, when you click ‘Save’ this will be highlighted with the reason. You will not be able to save until this error has been corrected (or date deleted).

16 Depending upon whether the patient required once, twice or no daily review will reveal a different set of sub questions below.

17 You only need to complete this section if the NEWS score triggered the need for a response.
The patient section allows you to save patients without all their details completed, but once you have completed the audit you must complete all relevant fields otherwise you will not be able to approve the record for submission to NHS England
