3 Strategic Plan Where? Why? Obvious need for resources
Lake County, Michigan Why? Obvious need for resources When researching the state of Michigan and comparing different counties and the prevalence of diabetes, Lake County stands out as one of the counties with a high percentage of adults with diabetes. Fourteen percent of the adults in Lake County live with a diagnosis of diabetes, which is almost four percent higher than the total in the state of Michigan (District Health Department #10, 2015, p. 22). There is a need for education within this community and information is not readily available. This is the reason we have decided on a program to assist the diabetic population of Lake County in gaining necessary resources to manage this complex chronic condition.

4 Mission To provide the local diabetic population with the necessary education, counseling and valuable resources to maximize the quality of life for diabetics.

Encourage personal growth Control of own health We at Diabetic Education Services believe that our program and resources offer a focused, educational curriculum and we strive to enhance our client's knowledge and abilities regarding their own diabetes management. Our program fosters personal growth, positive attitudes, and self-confidence by allowing the client's to take their health into their own hands.

Encouragement of the community to attend educational sessions Bring more primary care providers to the program The goals of our organization include: -Support the community in efforts to prevent type II diabetes -Encourage residents of the community to attend support groups and educational sessions for diabetics and their families -Provide educational materials regarding prevention and management of diabetes to the community -Bring in more primary care providers to participate in the program for aiding in the prevention and/or management of diabetes -Increase the number of residents who live well with diabetes and effectively manage their disease to prevent or delay complications and improve their quality of life We would like to see an increase in these goals taking place within a two year time frame after the organization is up and running.

Branding the organization Gain support from PCPs for community involvement Community Education Implement evidenced based research Financial assistance to support organization The objectives are tasks we would like to takes place within the first six months of the start up of the organizations. The objectives include: -Establish a marketing team to develop the brand and image of the organization within the community -Gain support from primary care providers for encouragement of community involvement in the program -Improve community understanding of diabetes, the complications associated with the disease and how to effectively manage and/or prevent the disease -Facilitate the implement of evidenced-based research findings in to health care practices -Form a financial committee for creating a plan to appeal to donors and secure financial grants

8 Organizational Chart Diabetic Education Services starts with a Chief Executive Officer who will manage finances in addition to overseeing two different areas of the organization, Educators and Market and Fundraising. The educators will include a registered nurse with a diabetic certification and a registered dietician that specializes in diabetic patients. On the marking and fundraising side of the organization there will be a professional to lead each area. The following slides will include job descriptions for each department of the organization.

9 Chief Executive Officer
Head of Diabetic Education Services Minimum of Master’s in Business Administration Degree Minimum of 2 years experience A CEO is needed for head of Diabetic Education Services. Minimum education of a Master’s in Business Administration required and minimum of two years managing a non-profit organization. Strong grant writing skills preferred. We are seeking a positive, enthusiastic, and motivated leader for our organization. This person will be responsible for overseeing day to day operations. Encourages positive performance of employees in the different programs and educational sessions. They will be responsible for supervising the marketing, fundraising, and education aspects of the organization. Specifically, this person will help guide fundraising efforts such as isolating sources of funding and managing financial records.

10 Diabetic Nurse Educator
Minimum of Bachelor’s of Science in Nursing Degree Current license in the State of Michigan Minimum of 2 years experience Job Summary Essential Functions The Diabetic Nurse Educator will hold at minimum a Bachelor’s of Science Degree in Nursing and have a current registered nurse license in the State of Michigan. Must have a minimum of 2 years of licensed clinical practice. Current BCLS required. Experience in diabetic education is desirable. The nurse must be a self-directed professional with the ability to make independent decisions directing patient care. Demonstrates ability to use a computer with Windows format preferred. Job Summary: Will be providing professional nursing care and education services to people of the community. Development a curriculum on health which is based on evidence based research. And also focuses on diabetes education. Act as a consultant for healthcare therefore providing information on how to manage and care for diabetes (Dudley, 1980). Essential Functions: Promotion of the mission, vision, and values of the organization (Roosevelt General Hospital, 2013). Provide education related to diabetes and any diseases processes, medications, diet, nutritional monitoring, and any possible care of complications that may arise (Roosevelt General Hospital, 2013). Provide education and demonstrations on proper foot care, blood glucose monitoring, and administration of injections. Educates patients on the long-term complications that can arise without proper monitoring of blood sugar. Creates individualized treatment plans with each patient and includes frequent monitoring and revision the treatment plan. Documentation of the complete learning process between the educator and the patient. Provide consultations to doctors, and other staff members when needed. Provide and help coordinate community outreach educational classes to local community centers and schools (Roosevelt General Hospital, 2013). Works collaboratively with interdisciplinary team members.

11 Registered Dietitian Bachelor’s of Science Degree in Nutrition/Dietetics Current License in the State of Michigan Minimum of 2 years of licensed clinical practice Job Summary Essential Function The Registered Dietitian will have a Bachelor’s of Science Degree in Nutrition/Dietetics or related field, holds a current Registered Dietitian license with the State of Michigan, and has a minimum of 2 years of licensed clinical practice. Current BCLS required. Prior diabetic nutrition counseling preferred. Certification in diabetes education preferred. Job Summary: Will be providing a thorough assessment on the patient’s educational needs for dietary teaching in regards to diabetes. Will provide an education nutritional care plan focused on diabetic restrictions and requirements. Instructs outpatients on prevention methods, teaches diabetic diet classes, and conducts in-services for staff as needed. Essential Functions: Explanation of dietary and health needs for a diabetic. Explanation of the different sections of the food pyramid along with the standard requirements for diabetics to follow. Assist in developing diabetic friendly meal plans. Provide diabetic friendly options for snacks (NuritionED.org, n.d.). Works collaboratively with interdisciplinary team members.

12 Marketing and Fundraising Director
Bachelor's Degree 2 years experience Job summary Essential Function The Education Requirements for this position will include a Bachelor’s degree and 2 years of experience in management. This professional is to provide leadership in regards to marketing and communication of the facility. They will also oversee the organization of events to raise money, identify donors, and obtain donations. Strong computer skills are preferred so they can work with the CEO in maintaining budgetary reports and implementing policies to meet the organizations financial goals. They will be responsible in confirming that the organizations marketing is promoting and sending the right message to potential clients (Head of Marketing Job Description, 2015). A main responsibility will be developing plans for increasing the population’s knowledge of our programs and services. This person needs to be accessible, approachable, and able to problem solve easily and quickly.

13 Organization Budget Space to work Supplies Staff Payroll
Hours of Operation Client Payments To start, we would need to find a space to work out of which would include a rent payment and utilities (electricity, heat, internet and phones), equaling about $1,500 per month. An initial purchase of supplies such as computers, printers, copiers, office furniture and a fax machine is estimated at around $7,000, a one-time cost. Printer ink, paper, and general office supplies would be about a $200 monthly cost. In terms of staff, we find that the best combination of staff to meet our objectives would be a paid diabetic nurse educator, registered dietician and director of marketing and fundraising. Volunteers will fill in when needed to assist the director of marketing and fundraising. Full time employees will work 9:00am to 5:00pm Monday, Tuesday, Thursday, and Friday, 10:00am to 7:00pm Wednesday, and weekends and holidays off. The registered nurse annual salary will be $45,000 with an offered benefits package of approximately $8,000 and the registered dietician will be offered a salary of $40,000 with the same benefit package. The annual salary for the CEO would be $65,000. The director of marketing and fundraising will be offered a salary of $50,000 with a benefit package. There will be a fee for the services offered including individual counseling and support groups. Clients will have the option of obtaining information to provide to insurance companies to find out if the services are covered. Medicare and Medicaid typically provide coverage for diabetic education management services (Hess-Fischl, 2014)

Included in the advertisement are specific educational requirements, name of organization, location, and contact information. Places where this ad will be run are in Health Professional Jobs Plus (2009) with Lippincott Williams & Wilkins for a minimum of thirty days with prices started at $425 as well as with The Pioneer Group’s The Lake County Star (2017) for a minimum of one week with prices starting at $8.15 with an additional $0.25 per word over fifteen words. Additional sources where the ad will be placed are Indeed.com for free, with the option to pay to promote it (Indeed, 2017), and Monster.com for minimum of $399 for one ad location (Monster Worldwide, 2017) as well as with the District Health Department #10 (2017) for free.
