Select your required Individual Items here
If your practice is a branch please include your preferred surgery for collection here
Messages can be included here– if you wish to collect your items direct from a pharmacy indicate your choice here
Select “Submit Your Request”

7. Step 6 confirm You can Cancel, Change or Confirm your request here.
When you are satisfied with your request select “Confirm”
