Outline team-based strategies and emerging new models of care to improve outcomes for older adults with type 2 diabetes mellitus (T2DM)
Discuss factors that must be considered when setting glycemic goals for older patients with T2DM.
Outline strategies to overcome common barriers to suboptimal glycemic control in older adults with T2DM.
Recommend treatment adjustments for older adults with T2DM experiencing hypoglycemia.
Recommend treatment regimens for older adults with T2DM to maximize benefits for cardiovascular co-morbidities.
