Welcome and Overview The IQIN Peer Learning Teams are professional development communities for IN health center staff Practice Transformation Team ~ Chief Medical Officers, Directors of Operation, and/or clinical team leads. Health Information Technology (HIT) Team ~ HIT and EHR Specialists. Participants will effectively address on-the-job challenges and opportunities through peer-to-peer learning. Forums to discuss issues and concerns, and to clarify approaches, develop skills, and build capacity by sharing ideas and practices with other colleagues statewide.

Using HER and Pop Health Management Capabilities Social determinants of health Improved population health Using health information exchange Value, Efficiency & Effectiveness PCMH Workflows and Promising Practices The Practice Transformation Peer Learning Team focuses on the planning, management and execution of quality improvement initiatives to improve patient health outcomes. This team will also discuss how Indiana health centers are improving their health center operational and financial QI, and addressing emerging implications of value-based care on health center QI.

Benefits Discussion topics are specific, relevant, and customized to participants’ responsibilities, needs, and interests. Participants easily form bonds, and advise and inspire one another because they share many work-related interests. Conversations are cumulative, building on experience and knowledge gained across multiple topics. A summary of the insights and actions is provided to all team members. Summary notes remind participants about the full range of suggestions and ideas discussed.

Prepare: Spend time before the meeting thinking through the topics and bring any information requested. Actively Participate: Be fully present. Leave and work for after the meeting is over. Respect: Bring an open mind. Look for a range of potential answers and solutions to questions and challenges. Engage: Express yourself! Share thoughts and challenges with your peers. Confidentiality: Create a safe space for your colleagues to share.

Discussion What is the current composition of your health center’s care teams? For those who have PCMH recognition, how are you using care teams to support PCMH? Are there positions you would like to add or other changes that need to be made? What strategies does your health center use to standardize clinical measures and practices across all of your care teams? What health IT systems and reports are you using to support roles within your care teams? How does your health center foster a sense of belonging and meaning among the care team members to increase satisfaction?
