Adapted AMDOC Fitness Report Presentation

2 Overview The goal is to review the standardized fitness report format approved by the Deputy Surgeon General (DSG) in April 2013 and provide guidance in writing a fitness report.

5 Fitness Report Command Leadership Responsibility
The OHSU Bremerton Commanding Officer, Executive Officer and Senior Executives make every effort to ensure that each member’s FITREP is the best that it can be based on the input provided.

Accuracy is needed in continuity of career reporting Eliminate gaps in your Performance Summary Record Most officers in a ranking group will have the same dates for their Regular & Detachment of Reporting Senior Reports Dates vary commonly for the following reasons: Recently gained (Member's Report Date will be beginning of Period) Returned from deployment (Use Detaching FITREP from Deployment) Officer recently detached/transferred outside the command This is a Detachment of Individual FITREP and thus 1 of 1 *MANPOWER Item*

Should match narrative in block 41 (i.e. “Exemplary Leader” in narrative but 3.0 in leadership?) Block 40: Recommended Career Milestones (i.e. Executive Medicine/leadership positions/Service Schools) should be consistent with rank and specialty Do not put XO/CO for O-3s through O-4s

16 Block 41 Block 41: Write for your audience: Write for effect:
18 lines of text…write for impact Start with an attention-grabbing statement Focus on selling the particular officer Write for your audience: Different specialties within each Corps, other Navy Medical Dept. officers and line officers on the selection boards Do not use language or terminology that is only known to somebody in your specialty [JARGON!] Narrative should match traits in Blocks 33-38 Write for effect: Leadership and mentoring resulting in advancements/promotions, readiness, awards…

17 Block 41 Give selection board members the “power sound bites” to annotate OSR/PSR for briefing in the tank. Demonstrated leadership and mentorship Demonstrated growth through career and increasing responsibility FITREPs read like a book Superior performance: always going above and beyond, doing more with less, cost savings, use metrics. Remember context – put accomplishments in context so the reader sees the significance of the efforts Impact of officer’s performance on Command, Navy, Navy Medicine, DoD (i.e., LCDR Starboard is a member of HPC….means nothing, what did they do or accomplish?) Put leadership and performance into context that the board will understand.

18 Block 41 Opening: Hard-hitting opening statement, attention grabber. Most important contribution at the beginning of the narrative. Soft breakout if possible and deserved ( especially if 1 of 1’s). Specific accomplishments this period: ONE LINE OF WHITE SPACE Bullets: 3-4 cause and effect bullets with the most important listed first Impact bullets should support performance traits in blocks 33-39 Last bullet and only one statement on clinical sustainment or civilian occupation ADT/AT: 16JAN01-16JAN14, NAVHOSP PORTSMOUTH, INTENSIVE CARE UNIT *TO Item* Closing: End with promotion recommendation and recommendation for future assignments

19 Block 41 Example CDR Helm is my number one of all Commanders regardless of Corps. CDR Helm’s visionary leadership set the stage for transition from a strategic IRR to an operational IRR, fully integrated and aligned to support emerging manpower requirements. Specific accomplishments this period: -Oversaw the mobilization process of 235 Individual Ready Reserve (IRR) members from the Active Status Pool (ASP) in support of ongoing contingency operations yielding an overall mission completion rate of 96%. -Active member of two Lean Six Sigma process improvement projects reviewing the process’ of Line of Duty and Incapacitation Pay in PERS 95. -Refined the IRR Virtual Screening website on NSIPS, resulting in the improvement of IRR member’s completion of electronic mustering by 30% in less than one year. -Employed full-time at the University of Utah with the Health Sciences Dept. as a Health Systems Analyst. AT: 15DEC01-15DEC14, NAVPERS, RESERVE PERSONNEL MANAGEMENT DEPT *TO Item* CDR Helm has my strongest recommendation for promotion to Captain and executive assignments. (Always end with promotion recommendation and future assignments).

20 Block 41 Giving bullets context or “framing” for impact:
CDR Y. hand-selected by the CO to serve as OHSU Bremerton’s representative to the Expanded Multisystem Medical System (EMSM) Planning Group, developing a repeatable review process in executing the rollout of EMR to NOSC Kitsap -Sought out for expertise -electronic medical record system LCDR Z. is certified by the American College of XXX Only 10% of Navy XXX have earned this elite designation CDR Y. accomplished Medical Stand Down with a 70% cost savings to the NOSC compared to outsourcing 70% turned out to be $100,000 per year! Increased NOSC readiness from 88% to 94%

Do not use the numerical number for numbers less than ten Spell the numbers one through nine Do not use the symbols < or > Do not use the words about, more than, or approximately Use an exact number, if you want to state “more than 20” change the number to 21

22 Block 41 Reporting Seniors and Ranking Boards must clarify marks and also break out “Must Promotes” and “Promotable” officers. Remove doubt – be clear and concise if the officer is making significant contributions at the deckplate.

Member attached to the detachment 90 days or less Block 41 can be left blank if no significant accomplishments Add comments in block 41 to recognize: Award received Completion of school or course AT/ADT *TO Item* Comments are limited to two lines or less

25 Detachment of Member Detachment of Individual Report
The HQ FITREP Coordinator should be notified of any member transferring outside the command or mobilizing. A Detaching FITREP should be prepared immediately. If the member’s next assignment is known please include the information Assists Selection Boards in following record continuity
