Inclusive Education Referral Process
School Referral Step 1: Teacher has a concern about a specific student - they will discuss those concerns at their grade level meetings. Step 2: Teacher contacts the parents about concerns. Step 3: Teacher fills out a SIT Referral. Step 4: SIT Team reviews the referral and comes up with a plan. The plan will consist of the following; observations, rating scales, assessments, interventions, and suggested strategies. (This step can take 4-6 weeks). Step 5: The SIT Team reviews the referrals every two weeks. If initial concerns continue to be present the Inclusive Educator will contact the parents to discuss options. Step 6: If the student is evaluated and is diagnosed the Inclusive Educator and the team will develop a Student Education Plan. Inclusive Education Referral Process

Parent Referral Step 1: Parent contacts the Inclusive Educator about their child’s concerns. Step 2: Inclusive Educator does the following: contacts teacher about parent concerns, does observations, assesses students, reviews students records and administers rating scales. The Inclusive Educator will also fill out a SIT Referral and present student to team. Step 3: After completing Step 2 the Inclusive Educator will analyze all the information that has been gathered and will schedule a conference with the parent. Step 4: If the student is evaluated and is diagnosed the Inclusive Educator and the team will develop a Student Education Plan Inclusive Education Referral Process

A Student Education Plan, is an individualized education plan that the team develops to meet the academic, behavioral and emotional needs of the student.
