4 Team dynamics Successful team dynamics are critical during a multi-rescuer resuscitation attempt regardless of location. Good team dynamics may increase the chance of successful resuscitation You will need to know what to do but also how to communicate and perform as part of a team Learning objective: Describe the importance of teams in multi-rescuer resuscitation Perform as an effective team member during multi-rescuer CPR

5 Successful multi-rescuer team members not only have medical expertise and mastery of resuscitation skill, but also practice good communication and effective team dynamics which helps give victim the best chance of survival

7 Team Dynamics Video Segment (13 min)

8 Team Dynamics Roles activity.
You will work with your LAB groups Set up: Manikin on floor Each member picks a laminate Role Card (don’t look at card) Instructor will tell you when to begin Each person gets into place according to their Role Card around the manikin Once in place flip cards over and hold them up so the group can see Take turns saying what your responsibilities would be (help each other out) Perform 2 times.

12 Occurs when blood flow to part of the heart muscle is blocked
People often use the terms Cardiac arrest and heart attack interchangeable but they are not the same. Sudden Cardiac arrest Heart Attack Occurs when the heart develops an abnormal rhythm and can’t pump blood Results from an abnormal heart rhythm which causes the heart to quiver so it can no longer pump blood to the brain lungs and other organs It’s a rhythm problem Occurs when blood flow to part of the heart muscle is blocked When a clot forms in a blood vessel carrying oxygenated (oxygen-rich) blood to the heart muscle. No blood, tissue starts to die It’s a clot problem

13 Sudden Cardiac arrest Heart Attack Within seconds the person becomes unresponsive and is not breathing or is only gasping Death can occur within minutes if they are not treated immediately Signs may appear immediately or last weeks Chest discomfort Shortness of breath Cold sweats Nausea/vomiting Heart usually continues to pump blood Sometimes the blockage can lead to abnormal rhythm which can lead to sudden cardiac arrest Heart attack symptoms in women can be different Pain in the jaw, arms, back or neck Light headedness Nausea vomiting what happens?

15 CHAIN OF SURVIVAL A metaphor for the elements of the emergency cardiovascular Care (ECC) systems of care. There are 2 distinct Chains of Survival:

16 In-hospital cardia arrest (IHCA)
For adult patients who are in the hospital Usually caused by respiratory or circulatory conditions that get worse LINKS in the Chain of Survival for an adult who has cardiac arrest IHCA: Surveillance, prevention and treatment of prearrest conditions Immediate recognition of cardiac arrest and activation of the emergency response system Early CPR with emphasis on chest compressions Rapid defibrillation post cardiac arrest care

17 Out of Hospital cardiac Arrest (OHCA) We will focus on this chain of survival
Most cardiac arrests happen unexpectedly and result from underlying cardiac problems. Successful outcome of victim depends on early bystander CPR and rapid defibrillation in the first few minutes after the arrest

18 LINKS: Immediate recognition of cardiac arrest and activation of the emergency response system: 1st recognize that the victim is in cardiac arrest based on unresponsiveness, no breathing (or only gasping) and no pulse Once you recognize, activate the emergency response system or ask someone else to do it The sooner you activate the sooner the next level of care will arrive Early CPR with an emphasis on chest compressions If victim in cardiac arrest, begin high quality CPR without delay High quality CPR started immediately after cardiac arrest can greatly improve a victims chance of survival (bystanders should at least provide chest compressions)

19 Rapid defibrillation with an AED
LINKS Rapid defibrillation with an AED Rapid AED with quality compressions can double or triple the chances of survival Effective advanced life support Bridges the transition from BLS to more advanced care (IHCA) Post cardiac arrest care Once ROSC is achieved (they lived!) the next link is for the patient to receive post-cardiac arrest care (cardiac catheterization suite or ICE) to help prevent it from happening again

20 MAIN DIFFERENCES IHCA OHCA
Care depends on in-hospital system in place for surveillance and prevention Care depends on some on community and EMS providers support Care depends on smooth interaction between different departments within the hospital (physicians, nurses, therapists, pharmacists, counselors) Care depends on the Lay rescuers ability to recognize an emergency and activate the chain of survival. Care depends on available resources: the hospital will have all the equipment readily available (depending on facility) Care depends on available resources: may or may not have access to an AED
