OPTIC ATROPHY Dr. MANOJ VASUDEVAN MS, DNB, FICO, CORNEA & REFRACTIVE SURGERY FELLOWSHIP
2
WHAT IS OPTIC ATROPHY It is the condition used to describe the optic disc following degeneration of the optic nerve.
3
POA VS NORMAL
4
NORMAL VS OPTIC ATROPHY
5
TYPES OF OPTIC ATROPHY PRIMARY OPTIC ATROPHY: When the OA is due to any disease affecting the optic nerve but there are no ophthalmoscopic evidence of previous inflammation. SECONDARY OPTIC ATROPHY: When the OA is due to any disease affecting the optic nerve with a clear ophthalmoscopic evidence of previous insult of the optic nerve.
6
PRIMARY OPTIC ATROPHY
7
SECONDARY OPTIC ATROPHY
8
CAUSES OF OPTIC ATROPHY
PRIMARY OPTIC ATROPHY Multiple sclerosis Lebers disease Compressive lesions of orbit/brain Toxic optic neuropathy-Lead, Tobacco. SECONDARY OPTIC ATROPHY Papillitis/Optic neuritis Neuroretinitis Papilloedema
9
CLINICAL FEATURES OF OPTIC ATROPHY
Optic Disc of PRIMARY OPTIC ATROPHY- Pale & CHALKY WHITE, well defined margins and the background retina normal. Optic Disc of SECONDARY OPTIC ATROPHY- DIRTY WHITE, ill defined margins and background retina may be affected.
10
CLINICAL FEATURES OF OA
VA: Patient is blind in the eye with OA. PUPIL: Dilated not reacting to light, if partial and unilateral- Look for MARCUS GUNN PUPIL/RAPD. Early or partial atrophy- Color vision- defective, Visual fields- Central scotoma
11
SPECIFIC TYPES CONSECUTIVE OPTIC ATROPHY CRAO Retinitis Pigmentosa
CAVERNOUS OPTIC ATROPHY Glaucomatous optic atrophy
12
CONSECUTIVE OPTIC ATROPHY
RETINITIS PIGMENTOSA CRAO
13
FOSTER-KENNEDY SYNDROME
14
INVESTIGATIONS To find out the cause of the optic atrophy to prevent further loss in the same eye and safe guard the fellow eye. Sometimes may show a SOL of the brain and save the patients life. RADIOLOGY- CT/MRI SCANS!!!!! Investigations accordingly.
15
TREATMENT NO TREATMENT ONCE OPTIC ATROPHY SETS IN. BAD PROGNOSIS.
EARLY INTERVENTION AND TREATMENT OF THE CAUSE SHOULD BE ATTEMPTED.
