I don’t want you to have to wait on a call light later to go.” Pump & Equipment Position All equipment is functioning and out of Pathway? Q2 IV site checks Bed/Chair Alarms SCD’s Turn Q2 for at risk pts Reposition for comfort Bed Low and Locked Pathways Clear “Do you have everything you need within reach?” –Call light? Phone? Glasses? Possessions What is it? At least once every 2 hours, a designated staff member enters the room and address the 5Ps of patient care (above) Introduce yourself using unit consistent scripting Perform your scheduled tasks Address the 5Ps and any other comfort needs Prior to leaving, ask the patient, “I plan to be back in (time period), but is there anything else I can help you with now?” Why is it important? It can decrease falls by up to 60%1 Reduces pressure ulcer development 39% reduction in call light frequency2 Creates more time for patient care and charting2 Increases patient satisfaction
