Definition of Patient and Family Engagement
Patient and family engagement is a partnership among patients, family members and healthcare providers. This team provides knowledge, insight and viewpoints for: individual care the design, implementation and evaluation of programs and policies Engagement considers the values and needs of patients, healthcare providers and the community. Patient: intended to mean all clients, patients and residents Family: A family is a group of individuals who are connected by kinship, affection, dependency or trust. The family is defined by the client. (from Saskatoon Health Region)

Overview Not a legal document Meant to act as a personal communication tool To be completed primarily by the patient to enhance patient engagement, keep track of accurate history (facts, meds, surgery dates) and help the patient remember their care journey. Contains medical information to be shared between patients and any health care provider as desired / appropriate Includes tips on how to use the document
