September 2015 -- HHSC submits the 1115 Waiver Extension Request
(Waiver Term Ends September 2016) Continue the same funding for DSRIP and UC (DY5 DSRIP Funding and fund unmet UC) CMS could approve a 1, 3, or 5 year extension Asking for ALL projects to continue with a general progression Review the 3 Year Project Menu -- Those projects will continue Unless deemed high risk through the mid-point assessment We will be notified by June/July if project is under review Jan we will know if project is discontinued Projects not on this 3 Year Menu will not continue, project options: 2.4, 2.5, 2.8, 1.10 Also projects eliminated as high risk We can create a replacement project for the same value from an even narrower menu, and somehow build from previous project Waiver Renewal

Items to be considered and expect changes: Valuation # of Metrics Increasing QPI MLIU Requirements for a minimum percentage Services Improving Data Exchange Project Sustainability Planning Left Over Funds Performance Bonus Pool Allow Projects to be Combined into a Single Project Integrate DSRIP Efforts with Managed Care Medicaid Project extension plan

Each Anchor RHP website would have a “Stakeholder Resources” page, linked to /from HHSC site by May 15, 2015. This would include information on stakeholder opportunities, original RHP plans (which would also remain on HHSC website) as well as a description of the process by which 3-year projects were added in that RHP: RHP Organization Original Executive Overview with potential addendum update Original Community Needs Assessment with potential addendum update for 3-year projects RHP certifications (these may be signed after all updated information from HHSC and RHP Anchors is available for review) – HHSC will provide a template for certifications in early March Also, the projects will be grouped by Project Type; Secondary Type; Medicaid, LIU, or Both: For Example, Behavioral Health and Integrated behavioral/physical healthcare for Both Payer Groups Updated RHP Plans

HHSC Mid-Point Assessment
Meyers & Stauffer has started to review information reported for 3 year projects that did not have any information reported in April of Be aware that Meyers & Stauffer may contact providers with additional questions regarding any of the 3-year project that reported in October. Additional questions for 3 year projects will be coming in the near future. HHSC & Myers and Stauffer signed off on all Region 15 Change Requests and Plan Modifications April 1st, we will receive Myers and Stauffer results about the ‘reviews’ of our projects HHSC Mid-Point Assessment

April Reporting June 10th we will have results
July 2nd submit responses to NMIs Aug7th approve or deny NMIs July 8th IGT Due July 21st and 31st payments processed to providers April Reporting

Transformational Summary impact templates
We will send 2 Batches to HHSC: May 31st and June 15th Clinical Champions Workgroup will review: Identify and share promising practices with like projects around the state Inform the transformative impact of DSRIP projects and the development of content for the Statewide Learning Collaborative Support Waiver extension/renewal efforts with CMS Help inform ways to better evaluate projects in the next phase of the Waiver. Preliminary Survey is already posted on Website Another Survey will be posted in July once the draft Formal Renewal Document is posted HHSC will hold a DSRIP protocol webinar in August and discuss protocol changes at the August Statewide Learning Collaborative Summit tailored to the summit audience. HHSC will be here July 22 for a Public Meeting regarding the Waiver Extension/Renewal document 1 PM – 4 PM El Paso First Health Plans
