1 What you should know about ordering a Sleep Study

2 Referring a patient for a sleep study:

3 Always Use Ambulatory Referral to Sleep Center or REF99

4 Be Certain to Request Level of Service under prompt #1 : Type of Test

5 Identifying Type of Test when ordering a sleep test
Sleep study with post-study initiation of treatment- The patient will meet with an LVH board certified sleep physician who will both interpret and report sleep testing results to the patient and will arrange for any medical equipment (Eg. CPAP) or treatment needed. The referring physician will receive a copy of the sleep test report and plan of care for their patient records. Sleep study with interpretation only- The referring physician will receive the patient’s sleep test results that have been interpreted by an LVH board certified sleep physician. The patient will NOT consult with an LVH board certified sleep physician to go over the results. The referring physician will meet with the patient to report sleep test results and the referring physician will arrange for any medical equipment or further treatment needed for the sleep condition.

7 Epworth Sleepiness Scale (ESS)
Many insurances require an Epworth Sleepiness Scale (ESS) to help determine if the patient will qualify for an In-lab or home sleep test ESS results should be documented in the patient progress notes on the date the screening was completed Example: ESS=14 (it is not necessary to scan the report)

Go to patient chart Click more activities Click flow sheet (this brings you to encounter selection screen) Click search (far right side of screen) Click data base tab Type in Epworth Click IP Epworth (adults) or Peds Pulmonary (pediatrics) Click “Test” to begin screening Epworth tablets are also available for office use

13 In order for Medicare to consider coverage for PAP therapy Medicare requires the ordering physician:
Complete a face-to-face clinical evaluation in physician office within 6 mos prior to sleep testing Document the face-to-face evaluation in a narrative note in the patient chart Notes should include: Primary sleep complaints Signs and symptoms of possible sleep disorder History & Physical Physician’s recommendations for further sleep evaluation *If you are unsure of the required Medicare documentation please refer to LVPG Pulmonary Medicine at (610)
