Bene-Fit™ Hospital Indemnity / Health Risk Management Thank you for joining today. This presentation is typically only 20 to 30 minutes long, and I would like to be respectful of everyone’s time, so I will keep it brief and open up for questions at the end. However, feel free to create a dialogue and ask any questions you may have along the way. What I would really like to accomplish today is introduce this new product we have developed, what the back end looks like and what we are trying to accomplish, along with the unique value proposition that Bene-Fit has to offer to the employer and the employee. The first few slides we will run through are statistical information of what’s going on in the market place, which I am sure you are familiar with.

Bene-Fit is an innovative insurance policy with a health risk management program that provides you with better benefits. Our goal is to help you understand risk factors you may face and provide you with simple steps to help you on the path towards better health! As I’m sure you are all aware, the cost of health insurance in the United States has been steadily increasing over the last several years. Employers have tried to implement several different types of programs to help control these increasing costs while still providing the best possible health insurance plans for their employees. The Bene-Fit group has developed an innovative program to help employer’s manage costs while providing better benefits for their employees, simply through participating in a health risk management program. Our goal is to help you understand risk factors you may face and provide you with simple steps to help you on the path towards better health!

Coaches first meet with employees to discuss: Individual health risks and goals, Then create a personalized wellness program for each employee. All while helping the employee reach monthly compliance within the wellness program. Employees then get to select from a suite of fully funded benefits which include: Whole Life &Term Life Accident Cancer Dental & Vision Critical Illness Short Term Disability GAP

4 How Does Bene-Fit Work? You pay a pre-tax premium to fund the additional coverage. You get a personal health coach and participate in the wellness program every month. We issue you a claim payment to enjoy additional benefits without changing your monthly take home! So how does Bene-Fit work? 1) You will pay a pre-tax premium from your paycheck to fund the indemnity coverage. 2) Through your monthly interaction with your health coach, you will earn a claim payment. 3) These claim payments allow you to enjoy additional benefits with no change in your monthly take home.

This reduces their taxable income to $3, By participating in the wellness program, the employee will receive a claim payment of $ from the wellness policy at the same time as their regular payroll cycle. This creates $24.97 in weekly tax savings that is used to fund additional benefits with no change in the employee’s total take home!

6 What’s Next? You will have the chance to meet with one of our certified health coaches to create your employee profile. At this time you will be scanned by our non-invasive screening device called Sudoscan. You will then meet one-on-one with an enroller who will help you choose benefit products that are right for you! Please note: All information is private and protected. Only your health coach will have this information to track compliance. So What’s Next? As part of the enrollment process, you will have the chance to meet with one of our certified health coaches and complete our screening process with the Sudoscan device. This device only takes two minutes to complete the scan and is completely non-invasive, in fact you will feel absolutely nothing. This device help to detect early warning indicators of cardiovascular and diabetic risk. You will then meet with a licensed enroller who will not only explain your major medical coverage options, but help you to sign up for your additional fully funded benefits. The entire enrollment process only takes about minutes. All of your information is protected by HIPAA privacy and neither your employer nor your major medical provider is given your individual data. Only your health coach will have access to that information and their only objective is to help you understand your individual risk factors and very simple lifestyle changes you can make to have a significant impact on your future health.
