Two Types of FOAPAL Forms
Validation Forms – FTVxxxx These are query forms that enable you to view a full list (or a specific group) of each type of FOAPAL code. The same forms are also used as search forms from within many of the document creation forms. Maintenance Forms – FTMxxxx These are data entry forms that can be queried to see more detail about each code.

The FOAPAL query forms are: FTVFUND – Fund Code Validation FTVORGN – Organization Code Validation FTVACCT – Account Code Validation FTVPROG – Program Code Validation FTVACTV – Activity Code Validation FTVLOCN – Location Code Validation

FOAPAL Maintenance Forms - FTMxxxx
The FTMxxxx maintenance forms can be queried to display additional information about the code: FTMFUND – Fund Code Maintenance FTMORGN – Organization Code Maintenance FTMACCT – Account Code Maintenance FTMPROG – Program Code Maintenance FTMACTV – Activity Code Maintenance FTMLOCN – Location Code Maintenance

Index Form - FTMACCI The Index code is not a FOAPAL element - it points to a Fund, Org, and Program combination. The FTMACCI form is a maintenance and query form for Index codes used to find the FOP (and sometimes L) elements that are associated to an Index code. FTMACCI can also be used to find the Indexes that point to any FOP element.
