3. HDM DRIVER Form Introduction
In order to improve DUA Request Process, ICT Dealer has created a new standard template for User administration requests
The standard form has also some extra- sheets for some specific applications (like ALL POWER Catalogue, SAP, ASIST, and so on)
It is mandatory for the requesters to fill in correctly and accurately the form
You can find the step to follow to fill the new standard template in the next slides

4. Select the Language from the drop-down menu and click OK

The requester must fill Mandatory fields
The requester must leave blank field User ID
for a new brand user ID

6. Insert an “X” to mark the application desired

After inserting the “X” click on the button and will appear a specific work-sheet for each application where the requester has to insert the additional information needed 3
