Specialized Surgical Instruments & Techniques Endoscopy and Laparoscopy CTVT pages 1154-55
Objective: Properly identify specialized instruments and all of their parts, and understand the set up, care, and maintenance of each.
2
Endoscopy The technique of examining internal anatomy using fiber optic instrumentation View internal body structures via an orifice ; not an incision Use: examine tissues directly; remove foreign bodies; biopsy organs Usually under general anesthesia
3
Common Endoscopic Procedures
4
Types of Endoscopes 1. Flexible Fiber-optic Endoscope
Eyepiece for direct viewing by the operator OR screen only Can attach a camera to eyepiece for others to observe
5
Types of Endoscopes Flexible
7
Endoscope Capabilities
4 way distal tip deflection Water flushing channel Air insufflation channel Suction power Accessory channel
8
Types of Endoscopes 2. Rigid Endoscope
For procedures involving a direct pathway that are better viewed with a straight line of sight Otoscopy, rhinoscopy, cystoscopy, *arthroscopy, *laparoscopy
9
Endoscopy Preparation
Veterinary technician is responsible for prep: Hook up endoscope properly Attach to power/water source Enter patient data on computer Turn on machine and check light source Test the camera (if you have one) Set up ___________ water for flushing Test machine Press the water valve Submerge tip, cover the air valve, check for bubbles Leave tip submerged in water and press suction valve
10
Endoscopy Room Store endoscopes in a designated
area in a hanging position Safe keeping; longer life; allows proper drying Do not store in original padded case

Handling the Endoscope
15
Biopsy Sampling Flexible biopsy forceps obtain the sample
1.8- to 2.4-mm cup for specimens Use 25-27g needle to gently get sample out of the cups Sample storage?
16
Endoscopy Accessories
Biopsy forceps, wired snares, wire baskets, pronged grasping forceps, cytology brushes
17
Colonoscopy Patient preparation:
Perform fecal sampling before beginning procedure Fast for _______ hours; NPO for 4 hours Remove fecal material Lavage solutions given orally Lavage colon (enema)
18
Endoscope Cleaning Clean immediately with an enzymatic solution
*Never submerge hand piece!
19
Endoscope Cleaning All channels must be cleaned and then sterilized!
Flexible scopes: liquid sterilization Rigid: Liquid or gas sterilization
