Moved to St Marys Hospital, see separate sheet for phone/fax numbers. Solent NHS Trust Supply Service now. No other change. CD TTOs still dispensed in usual way.

Solent not legally allowed to supply. To be supplied from QA Hospital via ad hoc transport to Southern wards
Emergency ‘Crash’ boxes for ECT suite etc come from QA hospital
No change in ordering location. To still be supplied by QA and sent via ad hoc transport to wards
QA pharmacy will still provide this service. Hours and cut off times for service to be circulated
