7 Population Health Population health has been defined as "health outcomes of a group of individuals, including the distribution of such outcomes within the group". It is an approach to health that aims to improve the health of an entire human population.

16 Leadership Leadership Across the Region - At a minimum, this should include representatives from public health, healthcare, and other key stakeholders who are strongly invested in the affected population. Reports to reference: Collective Impact Common Table Health Alliance

Communication and Needs Assessment Elements 3 and 4 focus on creating audience specific communications, and assessing community needs. Each element is described: what it is why it is important and how it can be done with examples and references.

Priority topic or topics are identified through shared planning, assessment, and decision making. Measures and Performance Targets Public health measures exist through Institute of Medicine, 100 million healthier lives etc… ESRD measures will be more specific to population but can include important health measures for all ie smoking cessation Joint Reporting on progress Transparent, all stakeholders, timely, and easily accessible

20 Scalability Scalability – The ability to expand the initiative to other regions or groups. Assessed with project initiation, can expand with new discoveries through initiative Example – CMS State Innovation Models Initiative fosters testing and development of state-based models for improving health system performance through multi-payer payment reform and other system changes.

21 Sustainability Sustainability – The ability to stay productive over time. The ability to motivate structural changes can increase the likelihood that you will sustain the gains you make Georgia Health Policy Center Sustainability Framework - Identifies components that contribute to organizational and programmatic sustainability.
