Overview Using a Collective Impact framework and Partnership Alignment principles, we have engaged our community in two large scale transformation change initiatives: PMH/PCH – Phase 1. Frail Seniors – Phase 2. Social Determinants of Health Housing First initiative to support shared MHSU strategic priorities. These initiatives support broader health system priorities established by our Collaborative Services Committee Strategic Plan with special emphasis on key partnerships with local First Nations communities and Health Network. Personal Health Record Prototype – Original proposal, current expansion proposal

Why this approach in Cowichan? Alignment.
The Cowichan Collaborative Services Committee Vision: Make Cowichan a healthy community. The Cowichan Collaborative Services Committee Mission: To co-design solutions to improve population health, quality and patient safety, patient experience, and sustainability in the Cowichan Valley. The Cowichan Collaborative Services Committee Mandate: Partners co-determine priorities with consideration for alignment to Ministry priorities, calling on additional voices from patients and the community.
