Participation is voluntary
All residents eligible for outreach, case management and service coordination
Residents who are frail are eligible for supportive services
Frail = Deficits in 3 or more Activities of Daily Living (ADLs)
Professional Assessment Committee (PAC) reviews assessment to approve support services plan

Costs:
Reimbursement for Service Coordinator (.35 FTE)
Reimbursement for supportive services at 75%
Resident pays for supportive services at 25%
Contract rates for supportive services are $18 to $25 per hour
Annual budget for 2017 = $37, (broader uses of funds)
Individual resident services capped at $2,000 per year
