Average age = Minimum age for occupancy = 55 48% Male and 52% Female 56 single adults and 2 married couples Average income is $12,807

HUD’s Congregate Housing Services Program (CHSP) offered at two other LHA locations---Area Agency on Aging is the CHSP grantee Resident Services Program developed to provide similar services

Services Provided by Area Agency on Aging Outreach, Case Management and Service Coordination Two Full-time Service Coordinators from Aging Partners (3 buildings, 269 apartments) Contracts with qualified providers for supportive services Personal Care Housekeeping Services Transportation using taxi vouchers

Resident Eligibility: Participation is voluntary All residents eligible for outreach, case management and service coordination Residents who are frail are eligible for supportive services Frail = Deficits in 3 or more Activities of Daily Living (ADLs) Professional Assessment Committee (PAC) reviews assessment to approve support services plan

Costs: Reimbursement for Service Coordinator (.35 FTE) Reimbursement for supportive services at 75% Resident pays for supportive services at 25% Contract rates for supportive services are $18 to $25 per hour Annual budget for 2017 = $37, (broader uses of funds) Individual resident services capped at $2,000 per year

39 residents receiving service coordination 18 of 39 at high risk for a higher level of services but able to live independently with supportive services and service coordination

Additional Services: Medicaid and Medicaid Waiver Services Sliding Fee Scale for those without 3 ADL deficits Discretionary Fund with Area Agency on Aging Medicare – Assistance with Parts B and D Assistance applying for benefits: SNAP, SSI, etc. Assistance with Medication Setups and Management

Additional Services: Nebraska Telephone Assistance Program Emergency Response Systems Legal Services Education Programs Holiday Programs Consumer Credit Counseling Downtown Senior Center and Meal Program across the street Etc. Etc. Etc.----Many services provided through relationship with Area Agency on Aging

Outcomes: High level of acceptance of the program Support to allow individuals to live independently Access to more services and supports through relationship with Area Aging on Aging Lower cost services---estimated savings in federal Medicaid dollars for was $123,445; estimate for was $163,281
