4. Eligible Facilities Community Grants Program
Goal – benefit maximum sensitive persons per Port dollar
Children
Daycare, school, after school program, youth or recreation centers, centers primarily serving children
Elderly
Skilled nursing, assisted living, adult day care facility
Chronically Ill
Hospital, clinic, hospice facility
Similar facilities licensed under CA Health and Safety Code
Community centers designated as senior center by a public agency
Note not to serve individuals/homes

Project able to be completed by September 30, 2019
Previously funded projects with outstanding funds from Port-funded HVAC/filter projects not eligible to apply
