2 Required Documentation
The following forms must be on file prior to student attending school: Proof of age documentation - Student must be 4 years old on or before September 1, Original copy of birth certificate with raised seal Proof of address documentation Utility bill, Lease agreement, Notarized letter Cell phone bills and cable bills are not acceptable forms of documentation

3 Required Documentation
The following forms are due on or before Friday, September 2, 2016 (30 calendar days) Pre-K registration form Roster form with required Category 2 documentation Copy of Social Security card Immunization form 3231 Eye, Ear, Dental, Nutrition form 3300 Failure to send in all required documentation will result in student withdrawal.

4 Attendance Policy Students are required to attend a full school day (8:15a.m.-3:15 p.m.) to attend Pre-K Students are considered tardy at 8:40 a.m. Chronic or excess absenteeism and/or tardiness will result in the following process: Warning letter Withdrawal notification

5 Transportation Parents will notify teachers of the primary transportation plans for the child. The designated plan will be followed each day. If transportation plans ever differ from the primary plan given to the teachers, parents MUST provide a WRITTEN NOTE (including transportation changes, date(s) of changes, student’s name, parent signature) for the change to take place. A phone call will not be sufficient.

6 Transportation Bus Information
All in-zone parents should find out assigned bus numbers for their child, regardless if they utilize school transportation or not. Car Riders Parents must remain in their vehicle in the pick-up line. Students will be delivered to the car line when their designated vehicle pulls up. Early Pick-up Parents must park in the side parking lot if picking student(s) up early.

7 Work Sampling Online (WSO)
WSO is the online documentation portal that will be used to monitor and record student progress throughout the school year. Using the data input into the system, teachers will conduct two parent conferences (mid-year and end-of-year) to discuss any pertinent information. Parents will receive a narrative summary at each of these meetings that summarizes the information found in WSO.

8 “A Big Day for Pre-K” and Family Space
Pre-K has adopted the “A Big Day for Pre-K” curriculum for the school year. The curriculum website has an area that parents may log into that offers the opportunity to the following: Keep informed about classroom learning Read weekly tips for extending learning beyond the classroom Visit BookFlix to build literacy at home Anticipate developmental milestones with the Kindergarten Readiness Indicators Download books and educational resources

9 Required Materials FULL SIZE Book bag (needs to fit school supplies, folder, blankets, coats, etc.) Two sets of back-up clothing (Shirt, Pants, Underwear, Socks) Nap Mat Cloth bag with handles that mat fits in Fitted sheet for mat Blanket to cover up with Daily Folder (will be given to each student by their teacher)

10 Wish List There are many supplies that are vital to maintaining a healthy classroom that are not provided by the school. We are asking that parents help provide these items as they are able. WE CAN NEVER HAVE TOO MUCH OF ANY OF THE FOLLOWING ITEMS: Kleenex Hand Sanitizer Band-aids Baby wipes Gallon-sized Zip-Loc bags Sandwich-sized Zip-Loc bags Lysol Disinfectant wipes Large Trash Bags
