A story about something we learned since CHILA1….
Focusing on abundance works! We had been stuck trying to bring together various groups and committees into one, dynamic force for change. Instead of focusing on who wasn’t interested, we instead build upon those who were and we not only reached our goal but our end product was better than we could have hoped for.

What we learned about engaging the BROADER community in developing our aim
With a strong ‘backbone’ organization, and committed partners, you can hold a coalition together even if you begin with a narrow project focus. Organizations can be partners on some projects and obstructionists on others – have to learn to manage and keep moving forward

Our aim for SCALE is….. By the end of fiscal year 2017, Summit County will implement the Pathways Community HUB model with the capacity to serve 150 at- risk pregnant women in the Central and West Akron Clusters. We plan to improve equity by ensuring that our HUB begins in our most prioritized communities, addressing both the clinical and social determinants of health.

Potential Care Coordinating Agencies understand the HUB model and its impact on infant mortality rates Clearly defined contracts and agreements outlining a network of care coordination spanning medicine, social service, and public health Aim: By the end of fiscal year 2017, Summit County will implement the Pathways Community HUB model with the capacity to serve 150 at-risk pregnant women in the Central and West Akron Clusters. Outcome Measures: 1. Reduction in infant death before 1yr/age within cohort 2. # of completed pathways vs. incomplete pathways 3. Achieving national HUB certification A 501(c)3 within the community, who is trusted and knowledgeable about health and health disparity, steps forward to seat the HUB Healthy Connections Network reorganizes to seat the HUB in a neutral fashion that is trusted by the community at-large. At-risk pregnant women feel comfortable with a care coordination system that will meet their needs in a culturally competent manner. Robust client enrollment in the HUB from priority census tracts occurs. Payors recognize the value of the HUB model and support the HUB through reimbursement. Ohio’s Medcaid MCO’s contract with the HUB to reimburse for completed Pathways, helping the HUB sustain itself long-term

Successful completion of Ohio Commission on Minority Health Grant (due Sept. 28) Execution of contracts between our HUB and Care Coordination Support Systems Execution of contracts with Ohio Medicaid MCOs for reimbursement of pathways Execution of MOUs with participating Care Coordinating Agencies Hiring of HUB staff By end of fiscal year 2016, complete national certification process for the HUB By end of fiscal year 2016, identify and support 50 pregnant women in Central and West Akron through the HUB By the end of fiscal year 2017, identify and support 150 pregnant women in Central and West Akron through the HUB

What we learned from “Switch”
Rider: Our rider was telling us that chronic disease and older adults should be the initial focus of the HUB Elephant: While our elephant (and our elected officials) were telling us to focus on infant mortality Path: Our pathway was, in some respects, set for us by state officials but in the end we aligned our rider and elephant by creating a timeline and a “Pathway” to accomplish both

Top two things we learned from other communities since CHILA1
The diversity of partnerships Each community demonstrated multi-sectoral coalitions that draw from all types of industries and expertise. Others have “focused on abundance” and it has worked So many times we have had fits and starts on implementing an idea because we focused on what wasn’t working. CHILA communities all have track records of doing the opposite and making it work.

Our biggest challenge(s) right now are…..
Making sure all necessary voices are at the table AND on board with the plan Very competitive health care environment right now in Summit County. All three health systems are fighting with each other and it presents a challenge to ensure the project moves forward. Assuring partners that all will be served by the HUB The Pathways HUB model can help improve many of our challenges in coordinating services for individuals. We always need to make sure we don’t lose partners because our initial focus is not theirs.

Implementation strategies and ideas We are looking for innovating ways to implement this model for infant mortality while also building in the ability to SCALE up rapidly for chronic disease and older adults Fellowship We could use some soul rejuvenation to ready ourselves for implementation
