Once installed, the PC Gameplan application icon will be on your iPad or iPhone’s home screen. Click on the icon to begin application.

Using the PG provided ID and Password, click here to login into PC Gameplan iPad.

Click this button to start the survey.

11. Input Survey’s Contact
Click here to select Product for the listener (if available).
After inputting information, click on “Submit”
First Name, Last Name, and Gender are required information.
