PAIN MANAGEMENT
2
OBJECTIVES UPON COMPLETION OF THIS LESSON, YOU WILL BE ABLE TO:
RECOGNIZE THE TYPES OF PAIN AND HOW PAIN IS TRANSMITTED EXAMINE THE VARIOUS TECHNIQUES USED FOR PAIN RELIEF
3
ENGAGE WHEN WAS THE LAST TIME YOU WERE IN PAIN AND HOW DID IT FEEL?
WHAT WOULD YOU HAVED SOMEONE TO DO ABOUT IT?
4
KEY POINTS PAIN IS AN UNPLEASANT SENSATION, USUALY ASSOCIATED WITH DISEASE OR INJURY
5
TRANSMISSION STIMULATION OF NEURONS (PAIN RECEPTORS) NEUROTRANSMITTERS
SUBSTANCE P PROSTAGLANDIN
6
BRAIN – THE IMPULSE GOES TO THE THALAMUS, CORTEX, AND LIMBIC SYSTEM, WHERE IT IS PROCESSED
8
PAIN PERCEPTION - THE CONSCIOUS EXPERIENCE OF DISCOMFORT
GATE CONTROL THEORY - ONLY ONE IMPULSE IS TRANSMITTED AT A TIME ENDOGENOUS OPIODS – AN OPIOD LIKE SUBSTANCE FORMED IN THE BODY. EXAMPLE: ENDORPHIN
10
TYPES OF PAIN
11
CUTANEOUS PAIN – DISCOMFORT THAT ORIGINATES AT THE SKIN LEVEL
CUTANEOUS PAIN – DISCOMFORT THAT ORIGINATES AT THE SKIN LEVEL. EXAMPLE: BURNS VISCERAL PAIN – DISCOMFORT ARISING FROM THE INTERNAL ORGANS
12
NEUROPATHIC PAIN - NEUROPATHIC PAIN IS A COMPLEX, CHRONIC PAIN STATE THAT IS USUALLY ACCOMPAINED BY TISSUE INJURY. WITH NEUROPATHIC PAIN, THE NERVE FIBERS THEMSELVES MAY BE DAMAGED, DYSFUNCTIONAL, OR INJURED. THESE DAMAGED NERVE FIBERS SEND INCORRECT SIGNALS TO OTHER PAIN CENTERS. THE IMPACT OF NERVE FIBER INJURY INCLUDES CHANGE IN THE NERVE FUNCTION, BOTH AT THE SITE OF THE INJURY AND THE AREAS AROUND THE INJURY. ONE EXAMPLE OF NEUROPTHIC PAIN IS CALLED PHANTOM LIMB SYNDROME.
13
ACUTE PAIN - ACUTE PAIN BEGIN SUDDENLY AND IS USUALLY SHARP
ACUTE PAIN - ACUTE PAIN BEGIN SUDDENLY AND IS USUALLY SHARP. IT SERVES AS A WARNING OF DISEASE OR A THREAT TO THE BODY. CHRONIC PAIN - CHRONIC PAIN PERSISTS, DESPITE THE FACT THAT AN INJURY HAS HEALED. PAIN SIGNALS REMAIN ACTIVE IN THE NERVOUS SYSTEM FOR WEEKS, MONTHS, OR YEARS. PHYSICAL EFFECTS INCLUDE TENSE MUSCLES, LIMITED MOBILITY, LACK OF ENERGY, AND CHANGES IN APPETITE
14
CHRONIC PAIN CONTINUED: EMOTIONAL EFFECTS INCLUDE DEPRESSION, ANGER, ANXIETY, AND FEAR OF RE-INJURY. SUCH A FEAR MAY HINDER A PERSON'S ABILITY TO RETURN NORMAL WORK OR LEISURE ACTIVITIES. REFERRED PAIN – DISCOMFORT PERCEIVED AWAY FROM THE SITE OF STIMULATION. THERE ARE NETWORKS OF NERVES WHICH CAN CONDUCT PAIN SENSATIONS FROM THE ACTUAL SITE OF PAIN TO A MORE DISTANT LOCATION.
15
PHANTOM PAIN – DEEP PAIN IN TISSUES THAT HAVE BEEN SURGICALLY REMOVED ( AMPUTATIONS )
16
PAIN MANAGEMENT
17
DRUG THERAPY OPIODS – COMMONLY PRESCRIBED BECAUSE OF THEIR EFFECTIVE ANALGESIC, OR PAIN RELIEVING, PROPERTIES. MEDICATIONS THAT FALL WITHIN THIS CLASS INCLUDE MORPHINE, CODEINE, OXYCODONE, AND RELATED DRUGS.
18
NONOPIODS OFTEN EFFECTIVE FOR MILD TO MODERATE PAIN. PEOPLE DO NOT WANT TO BECOME PHYSICALLY DEPENDENT ON THESE DRUGS OR TOLERANT OF THEIR PAIN - RELIEVING EFFECTS. ASPIRIN AND ACETAMINOPHEN ARE AVAILABLE WITHOUT A PRESCRIPTION (OVER-THE-COUNTER OR OTC)
19
PLACEBOS A PLACEBO EFFECT OCCURS WHEN A TREATMENT OR MEDICATION WITH NO THERAPEUTIC VALUE IS ADMINISTERED TO A PATIENT, AND THE PATIENT'S SYMPTOMS IMPROVE. THE PATIENT BELIEVES AND EXPECTS THAT THE TREATMENT IS GOING TO WORK, THEREFORE, IT DOES.
20
SURGICAL APPROACHES ABLATION NERVE STIMULATION
21
NONDRUG / NONSURGICAL INTERVENTIONS
IMAGERY MEDITATION DISTRACTION/HYPNOSIS RELAXATION THERMAL APPLICATIONS T.E.N.S. UNIT
22
ACCUPUNCTURE ACCUPRESSURE- USES PRESSURE TO RELIEVE PAIN BIOFEEDBACK- A MEHOD THAT USES THE MIND TO CONTROL A BODILY FUNCTION THAT THE BODY NORMALLY REGULATES AUTOMATICALLY, SUC AS SKIN TEMP, MUSCLE TENSION, HEART RATE, OR BLOOD PRESSURE
