3 OUR MISSION To change Community Healthcore from a Medical Model to a Recovery Culture. “A process of change through which individuals improve their health and wellness, live a self-directed life, and strive to reach their full potential through a recovery support strategic initiative”.

Advancing Recovery Across Texas (ART) Implementation Grant (3 year) Now that the policies were completed—501C3 A Change Team was composed– 4 peers and 4 CHC executive department heads It served 3 purposes: To form a collegial relationship between peers and staff To implement the new policies and procedures recommended to CHC by peers To help Cornerstone implement the COSP by providing their expertise QI (Quality Improvement)/Data Collection Collect and analyze data Budgets, contracts, job descriptions, and more
