PATIENT CASE REPORT Acute Ischemic Stroke Follow-up
PATIENT PRESENTATION CASE IMAGES Age: 73 y/o male transferred from Southbay Hospital on 9/30/16 for possible Neuro Intervention at 11:21. Symptoms: On phone with son and sudden onset of headache, weakness both arms and legs, slurred speech. NIH Stroke Scale – 23. History: TPA initiated at 10:00am at Southbay Emergency Department. CTA at NS PATIENT CASE REPORT Acute Ischemic Stroke Follow-up PROCEDURAL DESCRIPTION Imaging: 9:30am - CT Stroke Alert – Acute CVA. No evidence of hemorrhage, mass effect, or obvious large territorial infarct at this time. 12:46pm – CTA/Perfusion completed at NS – No major branch occlusions. Small amount of penumbra on the peripheral portions of the Left MCA distribution. Plain Brain at Largo Brain Perfusion at NS CASE CONCLUSION PATIENT CARE TEAM Outcome: When patient arrived to Northside his NIH Stroke Scale was <2 with marked improvement. CTA/Perfusion ordered upon arrival which showed no evidence of acute large vessel occlusion and no significant perfusion deficit. TPA opened occlusion and no Intervention was needed. Patient was deemed stable for discharge home on 10/3/16.
