October 2017 Hello, eCommerce associates! Welcome to our 2018 Annual Enrollment briefing. We are excited to bring our newest eCommerce associates onto Walmart’s benefits during this year’s Annual Enrollment. Now that benefits Annual Enrollment is here, we want to walk you through all of your choices, and make sure you have what you need to decide which plans are best for you and your family. Today, we’ll walk you through important details about our benefits, and help you have a great enrollment experience. We’ll need your help in making this a smooth transition. You must enroll during our Annual Enrollment window in order to have benefits in We’ll talk more about this as we get into the briefing, but if you only take one thing away from us today, it’s that everyone needs to complete an enrollment session by November 10! 

2 Got benefits? 2018 ANNUAL ENROLLMENT IS OCT. 28 TO NOV. 10! Remember, you need to complete an enrollment session by Nov. 10 to get benefits in 2018. So mark your calendar, write yourself a note, or set your alarm, just don’t forget to enroll! To take advantage of Walmart’s benefits, you will need to complete an enrollment session. Annual Enrollment is the time you can pick benefits for The plans selected will become effective Jan. 1, If a medical plan is selected, you will receive a new plan ID card in the mail by the end of December. Please remember, this is the only opportunity to enroll in benefits for This is your chance to get the benefits you want. If you miss this opportunity, you won’t have another chance to enroll unless you have a status change event or during next year’s Annual Enrollment. Be sure to enroll by Nov. 10th.

First we’ll review your Benefits Eligibility. Then we’ll dive into our Medical Plans and talk about Other Health Benefits and our Financial Benefits And then close with an overview of the Tools and Resources available to you.

4 Benefits Eligibility Now, let’s take a look at who’s eligible for benefits and when you get them.

5 Full-time hourly associates Part-time hourly associates
Benefits eligibility Salaried associates Full-time hourly associates Part-time hourly associates Salaried associates are eligible to enroll for benefits between the date of your first paycheck and prior to the 60th day after your date of hire. Coverage is effective on your date of hire. Full-time hourly associates can enroll between the date of your first paycheck and the day prior to your effective date. Coverage is effective on the first day of the month of your 89th day of employment. Part-time hourly associates can enroll after one year – between your 52-week anniversary date and the first day of the second month following. Coverage is effective on the first day of the second month following your anniversary. At Walmart, we have three main associate groups. Salaried, full-time hourly, and part-time hourly. Our Salaried associates are eligible for benefits starting on their hire date and we give them 60 days to enroll. Our Full-Time Hourly associates are eligible on the first day of the month that contains their 89th day of employment. They have between their hire date and the date their benefits become effective to enroll Finally, our Part-Time Hourly associates can enroll in benefits after one year and their benefits start on the first day of the second month following their one year anniversary. YOUR SERVICE COUNTS! Some programs are available only after you’ve been with us for one year. If your organization was recently acquired, we’ll start counting from your most recent hire date at your original company.

For medical coverage, however, there’s an extra eligibility step. In order to be eligible for medical insurance, part-time hourly associates must work, on average, at least 30 hours per week.

8 Dependent benefits eligibility
All eCommerce associates are eligible for benefits. Want to cover your family too? Here’s who is eligible to be covered on your benefit plans: Spouse/partner FT/Mgmt associates may cover their spouse or domestic partner regardless of gender or martial status Please note that if you cover a domestic partner, your premiums may be treated as imputed income per US law. Check in with your HR associate if you have questions. Child(ren) and/or step-child(ren) All associates may cover their legal child dependents up to age 26. Includes children with spouse or partner. If you want to include your family in your coverage, you can! Salaried and full-time associates can enroll their spouse or partner. Keep in mind, if you cover your domestic partner, the cost of those benefits will be treated as imputed income and subject to taxes. If you have questions about imputed income, you should talk to your financial advisor. All eligible associates, salaried, full-time hourly, and part-time hourly, can cover their dependent children up to age 26.

9 Getting Started Now that we’ve covered who’s eligible for benefits, let’s get started on enrollment.

10 Things to know about enrollment
2018 Annual Enrollment Window Annual Enrollment takes place each Fall. Want benefits in 2018? You will need to complete an enrollment session in Workday by November 10, 2017. eCommerce Benefit Plan Year The Walmart benefit plans run Jan. 1 through Dec. 31 each year. Need a bit more information? Go online to Workday or WalmartOne.com/AEeComm to get even more details about the new benefit choices. New plans mean new ID cards If you sign up for a medical plan, your new ID cards will arrive at your home towards the end of Dec Here are some things to know about enrollment: Our Annual Enrollment happens each Fall. If you want Walmart benefits in 2018, you’ll need to complete an enrollment session in Workday between October 28 and November 10. The Walmart benefits plan year is from January 1 to December 31. Since you will be enrolling in Walmart Benefits for the first time, you will receive a Benefits Plan ID card at your home before December 31. Need more information? 12 Health Savings Account (HSA) Plan
2018 medical plan options You have access to two primary types of medical plans. eCommerce PPO Health Savings Account (HSA) Plan You have access to two types of medical plans. Sometimes three. The eCommerce PPO plan The Health Savings Account, or HSA Plan And depending on your location, a Health Maintenance Organization or HMO, All of which we’ll talk more about here in a minute

13 Key features of a PPO plan
Manage in-network out-of-pocket medical expenses and receive additional treatment options with a PPO plan. Deductible $300/associate $600/associate + dependents Out-of-pocket maximum $1500/associate $3000/associate + dependents Office copays $15 PCP $25 Specialist Additional benefits Chiropractic care Transgender benefit coverage Infertility treatment coverage With the eCommerce PPO plan, you will be able to manage you out-of-pocket medical expenses with Lower deductibles Lower out-of-pocket maximums, and Predictable and affordable copays for doctor visits. This plan will, however, cost you a bit more in payroll deductions. The plan also covers some additional treatment options like chiropractic care, transgender benefits, and infertility treatment.

14 Key features of an HSA plan
Build a tax-free personal health savings account for use with eligible health care expenses. HSA contributions Triple tax benefit Walmart match Walmart matches your contribution dollar for dollar up to $350 per year, or $700 if you cover dependents Using your HSA HSA may be used for any IRS-approved health expenses HSA dollars are yours to keep The Health Savings Account, or HSA Plan lets you build a personal health savings account. This is cold hard cash that follows you year after year and wherever you may go. Here’s how the HSA works: You can put money into your health savings account, tax-free, each paycheck Walmart will match your contributions dollar for dollar up to $350 per year, or $700 if you cover dependents. You can use your HSA money for most health care expenses without penalty or taxes. And the best part: it’s cash you get to keep, even if you change plans or leave the company.

15 HMOs available in select locations
Health Maintenance Organizations (HMO) Keep in mind, availability is based on the location you work, not where you live. Each HMO will supply plan details including prescription benefits. Depending on where you work, an HMO may also be available. HMOs offer convenient and coordinated care as well as predictable and affordable copays for doctor visits If a facility is eligible for an HMO plan, the HMO will send additional materials with the plan details to you directly

16 Pharmacy coverage All of the medical plans include pharmacy benefits.
PPO Plan Generic drugs: $10 / 20% out-of-network Brand-name drugs: $30 / 20% Specialty drugs: $30 / 20% HSA* Generic drugs: $4 Brand-name drugs: $50 or 25% of allowed cost Specialty drugs: $50 or 20% of allowed cost Prescription drug coverage is included with each of the eCommerce medical plans. For the PPO plan, generic prescriptions have a $10 copay, while for the HSA Plan it is just $4. HSA prescription benefits are only available at Walmart or Sam’s Club pharmacies. If you enroll in the PPO plan, you can get your prescriptions filled at pharmacies other than a Walmart or Sam’s Club at a higher cost. Please note, if the closest Walmart or Sam’s Club pharmacy is more than five miles away from your work location, you will be able to fill your prescription at any Express Script network pharmacy. SAVE MONEY. STAY IN-NETWORK! Staying in your plan’s network saves you money. So does visiting a Walmart, Sam’s Club or other network pharmacy. 17 Medical plan premiums After-tax Pre-tax
Associates’ share for the cost for PPO and HMO health plans is paid on an after-tax basis. This means Federal and state taxes are deducted prior to their premium being deducted. Pre-tax Associates’ HSA premiums are paid with pre-tax dollars from their paycheck. Depending on the medical plan you enroll in, your share of the cost of coverage will be paid on either an after-tax or pre-tax basis. PPO and HMO plans are paid after tax due to IRS regulations. This means federal and state taxes are deducted before your medical premiums are deducted. If you are enrolled in the HSA Plan, your premiums will be deducted before taxes.

When associates enroll in the eCommerce PPO or HSA plans, they get instant access to great benefits and programs that help them get the most from their medical plans. You can get peace of mind with Grand Rounds. Grand Rounds offers a second medical review, a written opinion, and a personalized recommendation, all at no cost to you. The process is quick and easy. Grand Rounds does all the work, including getting medical records. Grand Rounds also helps you find the highest quality doctors and specialists. Starting January 1, you can download the app, go online, or call and let Grand Rounds help you feel better about your health care needs. Grand Rounds is only available to associates enrolled in the HSA plan. Download the Grand Rounds app from iTunes or Google Play

20 Centers of Excellence offers:
Available only to associates enrolled in the HSA Plan Access to specialists for: Certain heart surgeries Certain spine surgeries Hip and knee replacements Medical review for breast, lung, and colorectal cancers with onsite visit if recommended Organ and bone marrow/skin cell transplants Weight loss surgery Centers of Excellence offers: Are you facing a serious health condition or surgery such as cardiac bypass, a spinal fusion, hip or knee replacement, breast, lung, or colorectal cancer, or maybe a transplant? Then the Walmart Centers of Excellence, or COE, program is here for you. COEs provide access to world-class specialists for these needs and more. And let’s face it, when it comes to serious health needs, we all want the best! The Centers of Excellence program is only available for associates enrolled in the HSA plan. It covers many conditions and procedures at 100 percent after you have met your deductible. And it even includes travel expenses for you and a care giver. In order to receive in-network benefits on the HSA Plan, you will have to use a Center of Excellence for spine surgeries and hip and knee replacements. Available only to associates enrolled in the HSA Plan Search for doctors and medical services nearby Easily compare prices Track health care spending and deductibles Access an online ID card Castlight lets associates: Are you interested in finding the best doctors, or comparing the costs of treatment? Castlight is a free online tool that lets you search for doctors and medical services and make decisions based on cost and quality reviews. And it’s personalized, so you can use Castlight to review your past claims as well as deductible, out-of-pocket, and even Health Savings Account balances. Starting January 1, if you are enrolled in the HSA plan, you can go online and register. Stay heathy by making the most of preventive care benefits with $0 copays at in-network providers. Annual physical Well woman exam Some lab work Preventive care includes: Stay healthy by taking advantage of our preventive care benefits. There’s no deductible and no cost if you use in-network doctors. Preventive care covers annual physicals, well-woman exams, annual child check-ups, and lab work. Help with: Grand Rounds Centers of Excellence 24/7 Nurse Hotline Nurse Care Management Healthcare advisors provide: Navigating health care, medical claims and treatment options can be confusing and just plain overwhelming. The health care advisor is available for personalized guidance on claims, billing, and understanding what programs are available to best meet your needs. You can find the phone number located on the back of your Plan ID card once you get it in December. Available only to associates enrolled in the HSA Plan 24/7 video access to doctors Video visits covered just like regular doctor visits Convenience at a lower cost Doctor On Demand provides: Skip the waiting room and see a doctor or behavioral health specialist without leaving your home with Doctor On Demand. This 24/7 service provides access to doctors online via video visits right on your computer or mobile device. It’s more convenient and costs less. It’s designed to compliment your medical coverage with 24/7 support, access to dedicated prenatal nurses, and lots of great stuff along the way. 28 Continuity of Care We understand transitioning to a new medical plan can be stressful, especially if associates are in the middle of care. If you are in the middle of specific care or treatment and your current doctor is not in the network, you can call your healthcare advisor to determine if it would qualify to continue care and treat your claims as in-network. 30 Preventive and diagnostic care: Annual maximum benefit:
Dental plan Annual Deductible: $75 per person $225 family Preventive and diagnostic care: 100% with no deductible for Preferred dentists Basic care: 80% after deductible Major care: 50% after deductible Annual maximum benefit: $2,500 per person Orthodontia: 80% after 12 months of coverage; $1,500 lifetime maximum The Walmart dental plan offers 100% coverage for preventive care. The plan also covers basic care, such as fillings, at 80%, after the $75 deductible. It covers major care, such as a root canal, at 50% after the deductible. And there is 80% coverage for orthodontia care once you have been enrolled in the plan for a year. There is a $2,500 annual maximum benefit to keep in mind. All associates can enroll for optional life insurance up to $200,000 for hourly associates, and up to $1,000,000 for salaried associates. The first $25,000 of optional life insurance is guaranteed, but if you’d like to purchase more, you may need to show proof of good health. You can also enroll for up to $1,000,000 of accidental death & dismemberment insurance coverage, which pays a benefit in the event you have a non-work related accident. 34 Critical illness and accident insurance
Critical Illness Insurance Pays up to $20,000 cash for specific conditions Accident Insurance Pays cash for out-of-pocket expenses if injured while not at work Sometimes you might find yourself in need of some extra cash due to a critical illness or accident. Critical Illness insurance will pay you cash if you’re diagnosed with certain conditions; up to $20,000. Accident insurance will pay you cash in the event of an accident outside of work, such as breaking a toe or needing an ambulance. With both these benefits, you can use the money for whatever expenses you may have. 35 Short-term disability for salaried associates
Salaried Short-Term Disability Provides 100% of associates’ base pay for up to six weeks after a seven calendar day waiting period, then 75% for up to 19 more weeks. If salaried associates need time off for a personal health concern, you’re covered by the company-provided short-term disability plan. It can replace 100 percent of your base pay for up to six weeks after a seven calendar day waiting period, then 75 percent for up to 19 more weeks. 36 Short-term disability for full-time hourly associates
Full-time hourly associates are eligible for short-term disability after one year. The paid amount of the STD basic plan is 50 percent of an associate’s average weekly wage, up to $200 per week, at no cost to them! If associates enroll in the STD enhanced plan, the paid amount is 60 percent of their average weekly wage, with no weekly maximum. OR The short-term disability [basic plan] automatically protects full-time hourly associates’ paychecks if you need to be away from work for an extended time due to your own medical condition. Associates in most locations are covered by the company-provided short-term disability insurance plan. Associates who work in CA, HI, NJ, NY, and RI have access to state-sponsored STD plans. Enrolling in the short-term disability enhanced plan can boost your benefit from 50 percent with a $200 weekly maximum to 60 percent with no weekly maximum (this plan is not available to associates in CA, HI, NJ, and RI). Both plans pay a benefit for up to 25 weeks after a seven-calendar day waiting period. Long-term disability Long-term Disability Picks up when short-term disability ends. Provides 50 or 60 (enhanced) percent of average monthly income up to $15,000 a month.
