To establish a Behavioral Health Crisis Stabilization Center providing a full array of crisis stabilization service to children and youth aged The center located on Parsons Academy Road campus would include reception, community care units, secure units centralized triage and crisis response team. It would also house four 5-bed units used for short term and intensive medium term crisis stabilization.
Saratoga Hospital

To renovate available commercial space at 24 Hamilton Street and expand and integrate primary and behavioral health services at that location.
The Addictions Care Center of Albany
$354,735
To fund the creation of The Behavioral Health Community Crisis Stabilization Center. The Center would house co-located primary and behavioral health services as well as care navigation, short-stay sub-acute crisis stabilization beds, ambulatory detox, and a wellness center.
The Center for Disability Services

To upgrade their current EHR to make it compliant with PCMH 2014 Level 3 standards.

To renovate unused space in Catskill’s Green Medical Arts creating a medical village with shared clinical and non-clinical space. Including primary, specialty and behavioral health services as well as PPS related care navigation, screenings, or training.
Equinox

To establish an integrated primary care and behavioral health care clinic extending a currently operating facility at 500 Central Ave. Integrated care would be maximized by a shared EHR.
