Majority of practices have issues with District Nurses not attending meetings
When nurses do attend – feedback that often do not know the patients

Practice nurses report lack of communication and feedback as key issue. Lack of communication to patients also an issue

Focus on devising and implementing care plan – away from tasks
Able to assess patients when unwell and appropriate response
Key team member at MDT meetings
Communication and call time standards
