What Can I Do On This Page?
How Do I View Address Information?
Click on the to the left of the address row to be viewed. That row will be highlighted.
Click on the button. The details for that address will display.
Click on the button to exit the page and return to the Address Information page.
Return to Step 1 to view another address.
Click on the button to return to the Health & Insurance Home Page.
View home address information.
View previously entered temporary or mailing addresses.
Create a new temporary or mailing address. NOTE: Retirees will also be able to update their home address through this page.
Did You Know?
Health plan eligibility (e.g., HMO options) are determined by the county identified in the Home and Work addresses.
Temporary Address should only be used when temporarily living away from home to be sure you receives any benefit related communications while away.
Temporary Addresses entered in this page will remain active for 90 days.
The links under Helpful Links can provide more information on a particular topic.
Important information and warning or error messages will display in the Message Area of the page.
How Do I Create A Temporary Address?
Click on the button. The Address Information Details page will display.
Enter the appropriate information in each field.
Click on the button. The temporary address will be saved and the Address Information page will display.
Click on the button to return to the Health & Insurance Home Page.
Select the address to be viewed and click on the VIEW button.
Click the CREATE button to add a temporary address.
