SGFP 22 Coupling On 5/26, identified excessive concentric offset for the 22 SGFP coupling (67 mils in horizontal direction) FMEA was developed which identified 9 potential causes Most likely cause determined to be turbine shifting on pedestal Next most likely failure modes considered were: Pump temperature causing thermal movement Pump shifting on base pedestal Pump case jacking bolts restricting pedestal movement

Loose Turning Gear Bolts LP bearing pedestal hold down bolt (found loose post event) Deformed dowel pin found 5/27 Dowel pin on opposite side found sheared

Inspection Timeline & Results
5/26 – Difficulty in installing bolting through turbine coupling 5/26 – Alignment checks post coupling found horizontal readings at 67 mil (vertical and axial acceptable) 5/27 – Found deformed dowel pin on east side of LP turbine bearing pedestal and sheared pin on west side 5/27 – Walkdown & Inspection by EIS and site personnel: Seal packing gap (0 mils on west side; 50 mils on east – should be centered) LP bearing pedestal appears to have moved west Other 5/27 walkdown results: Foundation has small crack in previous repaired location (acceptable – could be a pre-existing crack) Inspection beneath found no other issues (no loose parts, keys remain intact, etc)

Looking down, bearing pedestal appears to have moved west from this ridge Inspection results: Bearing pedestal visually appears moved to west Seal packing off-center to west Dowling pin found sheared on west, deformed on east Turbine coupling bolting difficulties Alignment checks off 67 mils in horizontal direction East Slide 1 view Turbine Pump West

Jacked turbine to align with dowel holes Performing alignment check to determine if further adjustments are required
