3 About us MercyCare is a leading Catholic provider of health, aged care, family and community services. We provide services directly to over 25,000 people and impact on the lives of over 100,000 people in the community each year.

4 About Mercycare MercyCare deliver services across three service directorates Aged Care Services – offering low care, high care, dementia specific residential aged care, home and community care packages, community nursing, respite, seniors day centre and retirement living in Mercy Retirement Village, Wembley. Family and Community Services – offering a wide range of programs including; Out of Home Care, Child Care, accommodation support for young people, FMHSS, Reconnect, Support for unaccompanied minors and families (asylum seekers) released into community detention, affordable housing for life and Community and Workforce Development.

6 What is MercyCare’s Stepping Stones?
Funded by Department of Social Services (DSS) A responsive service for children and young people up to the age of 18 who are affected by, or at risk of mental illness, and their families. The service identifies risk factors or issues which may lead to poor mental health outcomes for children and young people later in life. We work with children, young people and their families to address these issues, and strengthen proactive and positive factors. For children, young people and their families who are experiencing issues that are making them feel sad, upset or stressed. Discuss mental health stigma and able to call to check. If have niggling feeling please call and speak with one of the team and we can offer advice surrounding a possible referral. Free – support client – funded by DSS based in Mirrabooka mercy care community hub. Optional An outreach service that can come to you – also option to have client come to Mirrabooka site Fully Confidential – however confidentiality disclosure form can be signed to disclose certain information to organisations Flexible with appointment options – original service hours 9-9 been flexible adapted to 7-9

9 MercyCare’s Stepping Stones can Provide:
Short and long term case management for children, young people and their family with their concerns and issues with day to day living. Children and young people with a voice Development of children’s and young people’s social skills, confidence and social groups. Child and family orientated support. Short and long term support for children, young people and their family with their concerns and issues with day to day living. The service is Person Centred and will tailor support around the individual so we are very flexible. We can offer support to anyone who is at risk of developing a mental health condition. This could be bullying, stress, low mood, anxiety. We offer short term support being up to 6 sessions and long term support which can be weekly up until a year – with the possibility of extension if required. We also offer one of support and information. Have a no wrong door policy so will always link clients into alternative support services and follow up to ensure client is happy with signposting. Children and young people with a voice – supporting and listening to the clients needs Development of children’s and young people’s social skills, confidence and social groups Child and family orientated support – focussing on early intervention.

10 MercyCare’s Stepping Stones can Provide:
Children, young people and their families with information that will help them develop skills and strengths on their journey to health and wellbeing Links with other services to work holistically Group sessions Children, young people and their families with information that will help them develop skills and strengths on their journey to health and wellbeing – we do not offer clinical interventions we are case workers discuss job role. Links with other services – comprehensive networking database to ensure clients receive the most appropriate support option and reduce barriers to accessing services which are known which mental health services and diverse background. Group sessions – we are looking into running session in the future we are able to respond to the communities needs so please make us aware of services that may be required. We also aim to run groups to reduce the stigma of mental health and raise awareness in the community.

11 How to refer to MercyCare’s Stepping Stones Service:
Complete Referral form and to Call and discuss case with a staff member. Ask the client to call the service and self refer Complete Referral form and to – we can forward this electronically which can be forward to whole team. Call and discuss case with a staff member – more than happy to have a discussion surrounding a potential client Ask the client to call the service and self refer – we accept self referrals as well as referrals from professionals
