28/05/2018 Our Mission: ….. to provide a Nottinghamshire local offer of services and an integrated 0 to 25 multi-agency assessment process that leads to an education, health and care plan which is person centred and outcome focused, enabling the joint commissioning and provision of quality services which can be paid for using a personal budget.

New process: Education, Health and Care Plan Pathway

1 The Local offer SENCO’s will need to:
be familiar with the local offer be able to sign post parents to helpful sections of the local offer contribute information about the school as part of the local offer.

2 The Referral SENCO’s will need to:
be aware of thresholds and eligibility criteria for EHC plans be able to support and manage parents expectations submit requests for EHC plans on behalf of parents provide supporting advice and evidence when parents make the request themselves.

3 My Story SENCO’s will need to:
be able to support families in completing the “All About Me” provide professional advice about needs, educational outcomes and resources when requested by the EHC coordinator be familiar with the “WIKI” system of sharing information between families and professionals.

4 My Plan SENCO’s will need to:
contribute to Multi Agency meeting to help formulate the plan.

5 My Budget SENCO’s will need to:
provide information about financial contributions from school, families of schools and HLN provide a copy of provision map.

6 My Life SENCO’s will need to:
ensure that outcomes and actions specified in the EHC plan are carried out monitor progress support parents with development of the “WIKI”.

7 My Review SENCO’s will need to:
coordinate a person centred annual review need to feedback to the EHC coordinators the outcomes of the review meeting set new targets.

Parent Feedback …. liked the multi-agency approach
contained easy to understand language that described their child well felt fully included in the process schools have been very supportive better process that the Statement

Things parents said we could improve the process by …..
need to be kept in the loop at all stages we could to explain personal budgets better need to know who the key contact is who is the family supporter and what is their role.
