Nationality: The status of belonging to a particular nation by origin, birth, or naturalization. Residing at: address. Afflicted: affected especially by disease; suffering.

Hepatitis B: is an infectious illness caused by hepatitis B virus (HBV) which infects the liver and causes an inflammation called hepatitis. Hepatitis C: is an infectious disease affecting the liver, caused by the hepatitis C virus (HCV). Tuberculosis: MTB or TB (short for tubercles bacillus) an infectious disease caused by various strains of mycobacteria, usually Mycobacterium tuberculosis in humans. Tuberculosis usually attacks the lungs but can also affect other parts of the body.
