2 Service Array Priority Plan
Prioritize big picture areas for development over the next 3-5 years. Identify priority areas for development or enhancement in 1-3 years.

3 Goal A broad but specific and measurable statement describing the end goal. Children are healthy, safe, ready and successful in school and supported in quality and nurturing environments.

4 Emerging Priorities in [city/town]
Increased awareness of services among providers and consumers (211, Resource Guide, opportunities for learning and sharing among providers) Improved accessibility of services (need interrupters, materials, etc.) for non-English speaking families More 24-hour crisis services and services for “gap” Families and youth Alignment of employment opportunities and skills/training of workforce Better understanding of influx of young families and children (esp. in relation to impact on early childhood services) Transportation assistance Increase collaboration and coordination of services for families All Four Areas Basic needs Increase affordable housing options Attract new/more health providers especially for specialized services, i.e. pediatrics, psychiatry, etc. Support QRIS, Step Up to Quality, for all early childhood providers—Child and Youth Safety and Development Healthcare access and promotion Family life and development Continue to work on quality early childhood continuum, i.e. child care, preschool, home visitation, etc., to increase readiness for school Child and youth safety and development Involve families from the beginning to better understand needs

Potential for Family/Parent Support Potential for Sustainability

6 Priority Outcomes Changes or outcomes that support the achievement of the goal.

7 Policy issues needing attention
Priority Outcome Areas (small groups) What efforts/promising practices can we sustain, enhance or develop to improve our service delivery system? Policy issues needing attention Other patterns and themes Weaknesses Or gaps More info needed strengths

8 Legal and Policy Changes
Identify legal and policy issues which are affecting the prevention system. These may be areas a community can not change on its own.

9 Strategies: Where could we begin to focus our efforts/promising practices?
Areas which will take longer term planning or shifting of resources but which will have an impact on higher end systems of care May require developing components of a system (assessment, Family Centered Practice, coordination of agencies) May require capital investment, e.g., shared information systems

10 What do we really want to sustain?
Outcomes-Quality Improvements Systems and Systems Change Core Services/Access Sustaining change requires different actions and structures from the onset. Prioritize big picture areas for development over the next 3-5 years Not necessarily programs or grants Best Practices

11 Strategies: Short Term (1-3 years)
Priorities that need to be addressed within 1-3 years Possible to accomplish but will require some resources (time, talent and financial) Consider essential to prevention and Child Well Being outcomes

12 Strategies: Quick Wins
Areas that require no funding or for which the resources already exist Changes to actions, e.g., community education, shared resources for staff training, cross training people who already have contact with a certain consumer/client group to include another function

13 Actions Tangible resources/activities implemented/created by the project for the purpose of achieving strategies

14 Identify Impact/Measures
Improved Child Well Being Indicators Enhanced Protective Factors Contribute to Shared Vision
