Generalized, frontally predominant, rhythmic fast activity is the most common ictal EEG accompaniment of generalized tonic seizures. Ictal EEG patterns in asymmetrical tonic seizures reveal initial high-amplitude slow-wave transients or midline sharp waves followed by bilateral fronto-central low-voltage fast activity or electrodecrement. Electrodecrement usually evolves into low-voltage fast activity and then bilateral fronto-central or generalized rhythmic theta slowing.
