What do Trusted Assessors do?
Speed up existing processes to reduce delayed transfers of care One profession or service trusting the assessment of another profession or service and acting upon it. Promote safe and timely discharges There is no single model Doesn’t remove statutory responsibilities

What they are and what they are not
Models will vary depending on local circumstance Subject to the agreement of local partners The model does not remove the statutory accountabilities of organisations who sign up The model does not support the transfer of costs from one sector to another

Transfers from hospital to an existing support package Transfer of patients to an interim support package e.g reablement or intermediate care New admissions to residential care & New packages of domiciliary support Eligibility Assessments under The Care Act Assessments for occupational therapy equipment

CQC Guidance ‘“Where a provider is confident that they can rely on information from hospital or care management staff, and that on the basis of this information they are able to meet the person’s needs, they do not necessarily need to see them in person. This includes in relation to gaining consent to their care and treatment being transferred back to the care home.” CQC, October 2016

Getting started Consider how others have developed Trusted Assessor models eg: Tower Hamlets Medway South Warwickshire Sheffield Consider how strong the relationships and trust are locally between health and social care commissioners and providers Start engagement and Co-design with all stakeholders a streamlined process Remember trust is essential; an MOU might be useful in articulating the vision and mutual understanding

Getting Started (2) Agree who can be a trusted assessor and what kinds of assessment are for consideration and what is out of scope Ensure there is a process for patient and carer involvement Agree competencies for TA and put in place training requirements Agree where this could be put in place quickly to test

Evaluation Agree metrics to be used to monitor how the service is operating and its impact Build in a feedback loop/hotline into the model Consider how the trusted assessor model will support delivery of other interventions
